Licensing Program Analyst Albert Johnson arrived on 1/09/2025, unannounced to conduct an annual inspection. LPA met with Eva Gahob and explained the purpose of the visit.

The facility is a RCFE with a current census of 5. The facility has 4 bedrooms and 2 bathrooms. Three bedrooms are being used, one room is empty, but ready for a resident to use. The facility has a formal dining area and a formal living room. LPA conducted the inspection using the CARE tool. LPA and Eva inspected the physical plant including but not limited to the kitchen, bedrooms for residents; two resident bathrooms, laundry area, and backyard area. LPA observed the facility to be free of odor. LPA observed sufficient furniture and lighting throughout the facility. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Hot water temperature was measured at 112 degrees Fahrenheit, which is within the required range of 105 to 120 degrees. Fire extinguishers (Dated 9/7/24) and smoke detectors are current and in compliance with fire safety. LPA observed centrally stored medications (Locked in the kitchen cabinets) and toxin are kept locked and inaccessible to residents. LPA reviewed and compared resident medication vs. resident medication logs. LPA reviewed 6 resident and 2 staff files, including criminal record clearances. All staff are fingerprint cleared and associated to the facility. First aid kit was checked and is complete. No citations, advisories given.
