On 06/07/2022 at 1:25 PM, Licensing Program Analyst (LPA) T. White arrived unannounced to conduct a required 1-year annual inspection. LPA met with Caregiver, Angelita Yapo and later met with Administrator, Marivic Teano- Chua at 2:45 PM. LPA explained the purpose of today’s inspection and was allowed entry into the facility that is licensed to serve a total capacity of 15 residents, which one may be non-ambulatory.

A comfortable temperature is maintained at 75 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 106.8 degrees Fahrenheit. There is a minimum of 7-day supply of nonperishable and 2-day of perishable foods.

LPA observed smoke detectors are interconnected with the fire department. LPA observed sprinklers installed throughout the facility. First aid kit was observed to be complete. LPA reviewed 5 resident files and 5 staff record files.

LPA observed the following deficiencies:
- Fire Extinguisher was last serviced on May 27, 2021. Based on Fire Department, fire extinguisher should be serviced annually.
- LPA observed carbon monoxide detector was not present at the facility. Based on Administrator, the facility has not installed the carbon monoxide detector yet.
