13 | On 11-26-2025 at 10:30am, Licensing Program Analyst (LPA) Michael Bilger arrived uannounced to open and investigate the complaint allegation noted above. LPA met with staff member Maryann Patacsil (S1) and explained the purpose of the visit. Adminsitrator/Licensee Ernesto Patacsil, Jr. was made aware of LPA's visit and purpose via phone. During this investigation, LPA conducted interview with staff (S2) and S3. Additionally, LPA conducted a facility observation and reviewed pest control service records from June 2024 to current. Based on interviews and observations, it was determined that a presence of rodents was noted on or about Septemeber 2025. A review of pest control services revealed services for September 16, 2025, and indicated targeted pests to include, but not be limited to: "Mice/rats." Pest control services have continued until current date based on record review. LPA observed bait boxes placed throughout outside of facility by pest control. Additionally, LPA observed record reviews to indicate additional sealing of facility by outside service as a function of pest control. As a result of this investigation, it is indicated that Licensee has taken adequate and timely steps to address a presence of rodents. Therefore, this allegation is UNSUBSTANTIATED.
