25 | Licensing Program Analyst (LPA) Ruth Wallace conducted unannounced required 1 year annual inspection visit. LPA met program director and explained purpose of visit. This day program is vendorized through Valley Mountain Regional Center (VMRC) to accept and maintain a 3:1 client to staff ratio at all times.
LPA and program director toured physical plant of facility. LPA observed that there was a single central room where all of the clients were held to participate in activities, arts & crafts, and discussions related to their scheduled itineraries. Office areas were observed to be adequately furnished and maintained at this time.
A tour of the exterior grounds was conducted. Area for client pick up and drop off were toured. Policies and protocol for receiving clients into this day program upon arrival were discussed with the Assistant Program Directors at this time. Tour of the facility kitchen was conducted. No meals prepared or completed for any of the clients while at this day program. This kitchen area is mainly used to store lunches for the clients in the facility refrigerator and the usage of the microwave and other kitchen items to heat up the clients' food. This kitchen area was made inaccessible to the clients.
The facility submitted a LIC 808 mitigation plan, which was approved. Water temperature reads 112.5*F in the kitchen sink which meets the 105-120 degree Fahrenheit regulation. and room temperature reads 74.5*F. LPA observed the facility to have adequate snacks. All activity and office rooms were sanitary and had the required furniture and furnishings. The facility common areas were clean and furnished. Smoke and carbon detectors were in good repair. Fire extinguishers are in compliance.
LPA reviewed five staff and five client files that were locked and readily available for review. Staff had current First Aid/CPR certificates All staff have criminal record clearance and are associated to the facility.
Per the California Code of Regulations, Title 22 no deficiencies were observed or cited.
Exit interview held with program director. A report and LIC 811 (Confidential Names) was left at the facility
