25 | On 1/19/23 at 11:12 am, Licensing Program Analyst (LPA) Chavez visited the facility to investigate a complaint. During the investigation, LPA learned that two staff had tested positive for COVID-19 on 1/12/23 and 1/14/23 and the facility did not report these events to CCL. LPA interviewed the administrator and Staff #1 (S1) and both said they had not reported it to the Department. They state they were unaware of the requirement to report. On 1/12/23, LPA spoke with S1 to obtain information on two residents who were COVID-19 positive on 1/7/23 and 1/10/23 and not reported to CCL. Staff failed to report an epidemic outbreak within the 24-hour reporting requirement.
Exit interview conducted, deficiency cited, and a copy of the report and appeal rights emailed to the administrator.
