On 7/23/24, at 8:45 AM, Licensing Program Analyst (LPA) Elvin Baddley conducted an unannounced Annual/Random Inspection of the abovementioned Family Child Care Home (FCCH). LPA met with Licensee Rhonda Evanoff and explained the purpose of the inspection. LPA, in the company of Licensee toured the interior and exterior of the FCCH. The FCCH’s dining room, living room, backyard, converted garage, upstairs bedrooms and hallway restroom are used for childcare services, while the remainder of the home is exclude. At the time of the inspection, LPA observed seven children (one biological) present along with an Assistant providing care and supervision.

Required documents and forms are observed predominantly posted in FCCH's converted garage. LPA observed a fireplace in the living room which is screened by glass. LPA observed a child safety gate at the base and top of the stairwell which serves as a barrier to the home's stairs. LPA observed the restroom used for care to be clean and free of toxins. The FCCH has combination smoke/carbon monoxide detectors throughout the home. LPA tested a combination smoke/carbon monoxide detector at the FCCH at 9:25 AM which was found to be operable. LPA observed a regulation fire extinguisher (2A10BC) in the FCCH which was service on 7/23/24. LPA reminded the Licensee to either service or purchase a regulation fire extinguisher annually. LPA reviewed the home’s fire/disaster drill logs. The most recent fire/disaster drill occurring on 7/2/24. The FCCH maintains operable telephone services.

The FCCH has ample ventilation to support the children’s comfort and safety. Medication and sharps are stored on an elevated shelf in a kitchen cabinet which is beyond the reach of children in care. LPA observed cleaning compounds stored underneath the kitchen sink which is secured by a child safety locks. Toys, furniture and equipment observed in the FCCH are age appropriate.

The backyard is enclosed by wooden fencing. The fence’s entry/exit gates are secure. The backyard also has three sheds which contains yard tools, equipment and supplies. The sheds were all secured by lock. The backyard contains a converted garage which is equipped with child appropriate toys and furnishing. (CONT. 809-C, Page 2)

Likewise, toys, equipment and play structures observed in backyard are age appropriate. No bodies of water are observed on site.

Children records are reviewed. The records are current and complete with children's emergency contact information as well other required licensing documents and forms. LPA also reviewed the Licensee’s records which are current and complete with Pediatric CPR and First Aid certifications (ESMA) expiring on 5/2/25, and Mandated Reporter Training expiring 7/23/26. LPA observed two dogs on site. The vaccination records for the dogs were reviewed and found to be current. A firearm and ammunition is on stored on site. LPA observed aforementioned and noted each is secured, stored separately and are in an excluded area of the home.

Licensee does not provide Incidental Medical Services (IMS). IMS policy was discussed. For IMS information see PIN 22-02 CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

During the exit interview, the Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS on 7/23/24.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee Rhonda Evanoff.
