This is an Amendment of the original document dated 7/28/22.

On 7/28/22, at 9:30 AM , Licensing Program Analyst (LPA) Elvin Baddley conducted an unannounced One Year Required Inspection the abovementioned Child Care Center (CCC). LPA met with Director Merideth Eades and explained the purpose of the inspection. LPA, in the company of Director, toured the interior and exterior of the CCC. The CCC operates from 8:00 AM to 5:00 PM, Monday - Friday. The CCC is a combination preschool with an infant program and uses three classrooms for child care. LPA notes the CCC submitted documentation to address a change of capacity. The inspection will address the aforementioned. LPA observed nine children on site during the inspection along with two teachers (cleared and associated) providing care and supervision.

LPA observed the CCC to be clean and organized. The classroom possessed age appropriate toys and furnishings for children in care. LPA observed operable carbon monoxide detectors in each classroom which was tested at (12:20 PM) and found to be operable. LPA observed the restroom attached to each classroom to be in safe and sanitary operating condition. Operable refrigerators are observed on site in each classroom with refrigerated food. The parents of children in care provide children with snacks and lunches. CCC staff ensure lunches and snacks from home contain items from the essential food groups and are void of junk food items. Children also have the ability to access filtered water inside and outside of the classroom. The Director was informed of the need to have the CCC's water sources tested for lead containments. LPA observed disinfectants, cleaning solutions, medications and sharps stored in elevated locations in classroom which are inaccessible to children in care and beyond their reach.

The CCC has two outdoor play areas. One is attached while one is in the front of the property. Each is enclosed. The outdoor play areas entry/exit gates are secured. LPA observed age appropriate toys and play equipment in the play areas. LPA observed the toys and play equipment in the play area to be in suitable condition and free of sharp, loose or pointed objects. The Director and was reminded to remove toy and play (CONT. 809-C)

equipment from the play area when the aforementioned begins to degrade and is not in good repair. The play structures in the areas have adequate cushioning to absorb falls and shade created by shade covers, trees and building overhangs. No bodies of water are observed on site.

LPA observed licensing required notices and documents posted predominately on the walls in each classroom. The CCC utilizes an electronic sign In/out. LPA reviewed the aforementioned and found sign in/sign out information contains the drop off and pick up times of children as well as the authorized representative.

Staff and children's records are reviewed. The children’s records were complete and found to contain emergency contact information as well as immunization records. Staff records were reviewed and contained current pediatric First Aid/CPR certifications as well as certifications for Mandated Reporter training. Director was reminded to ensure training certifications are current and updated prior to expiration.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility representative Merideth Eades,
