On March 13, 2024, at 1:05 PM, Licensing Program Analysts (LPAs) Francisca Velazquez and Joaquin Mendez conducted an unannounced Case Management- Incident inspection. The purpose of this report is to document deficiencies observed during today's inspection. LPAs met with Ana Rodarte, Licensee and advised her the purpose of the inspection. LPAs observed nine (9) children being cared by Licensee and Assistant.

Upon arrival, LPAs observed Licensee, assistant and six (6) children playing in the front outdoor yard.

At 1:06 PM, LPAs and Licensee walked into the home and observed three (3) infants napping inside with no adult supervision. LPAs note two (2) infants were napping in cribs and one (1) infant was napping on the floor.

At 1:48 PM, LPAs observed the door to the garage to be open. Licensee reported that the door is broken and therefore can be closed. Inside the garage, LPAs observed cleaning compounds and toxins that can pose a danger to the children in care.

At 1:49 PM, LPAs observed the cabinet under the kitchen sink has cleaning compounds and it is not lock, therefore the cleaning compounds are accessible to children in care.

At 1:51 PM, LPAs observed cleaning compounds under the children's bathroom sink that is not lock, therefore the cleaning compounds are accessible to children in care. Additionally, there were shampoos, conditioners and razors within reach of children in the shower.

At 1:52 PM, LPAs observed the fireplace in the living room is accessible to children in care.

During inspection tour, LPA observed accessible toxins in the garage, under the kitchen sink, under the children's bathroom and in the front play yard. Items observed were awesome cleaner, glass cleaner, pinalen, beach, detergent, Clorox, shave cream, cologne, shampoo, conditioner, razors, gas can, gardening tools and large wood stumps. This is a potential risk to the health, safety and personal rights of children in care.

At 1:53 PM, LPAs observed an infant bottle in the crib where an infant was sleeping when LPAs arrived.

At 1:55 PM, LPAs observed gas can, gardening tools, open trash can and large stumps of wood that were accessible to children in care.

At 1:56 PM, LPA observed a johnny jumper and a walker that was accessible to children in care.

While interviewing Licensee, it was disclosed that one (1) adult that lives in the home is not fingerprint cleared or associated to the FCCH.

While reviewing fingerprinted adults, it was discovered that Assistant is not fingerprint cleared or associated to the home. Both Licensee and Assistant disclosed that she has been helping the day-care since January 2024. Assistant is also not fingerprint cleared or associated. Licensee disclosed that she is in the process of getting fingerprinted. Additionally, it was disclosed that licensee does not have a file for Assistant for review.

Today, deficiency cited under Title 22 Division 12. Appeal rights given. Upon receipt of this report, licensee shall post and provide copies of this licensing report to parents /guardian of children in care at the facility and to parent/guardians of children newly enrolled at the facility during the next 12 months. Licensee to provide LIC 9224 for each child in care and have each parent sign the form that they have received a copy of the report.

Exit interview was conduct and report was reviewed with Licensee, Ana Rodarte. Notice of site visit was provided and must remain posted for the next 30 days.
