On 1/17/23, at 7:30 AM, Licensing Program Analyst (LPA) Elvin Baddley made an announced Prelicensing Inspection of the abovementioned Child Care Center (CCC). LPA met with Meredith Eades, Administrator of the CCC and explained the purpose of the inspection. There are no children on site at the time of the inspection. Administrator informed LPA of intention to operate the CCC from 8:00 AM to 5:00 PM, Monday - Friday. LPA notes the CCC is part of a combination CCC which consist of an Infant Program with a toddler option, a Preschool Program as well as a School Age Program (currently inactive).

The Infant Program consist of three buildings with three age groupings. The Slumber Room houses children 0-12 months of age. The Enchanted Forest room consist of children 12-18 months of age and the 100 Acre Wood Room consist of children 18 months to 3 years of age. Each room contains appropriate ventilation for child care. At the time of inspection, the rooms are outfitted with age appropriate furniture and equipment. Each room has operable carbon monoxide detectors which were tested and found to be functional. Medication within the each room is secured and placed in elevated locations. Likewise, cleaning products and toxins will be stored within secure cabinets. Sharps in the room are located in elevated locations outside the reach of children in care. Children will bring their own snacks, lunches and bottle water. The CCC afforded the children water treated through a reverse osmosis system. Water testing for lead containments has been completed. The CCC has operable refrigerators on site for refrigeration of children's food items when need be. The CCC has cribs (Slumber Room) and sleeping foam mats of appropriate thickness for children which have advance levels of mobility. Changing table are observed in arms length of sinks in the CCC and restroom are also available. LPA notes the restroom at the CCC are in sanitary operating condition. The Administrator informed LPA children from the 100 Acre Wood Room and the Enchanted Forrest Rooms are escorted by staff to the restroom area to ensure safety.

The outdoor area of the CCC is enclosed by wooden fencing. The fence entry/exit gates are secured by lock.

The area has ample shade provided by both trees and building overhangs. The footing in the areas are varied. The outdoor area is outfitted with age appropriate equipment. LPA observed a locked shed in the outdoor area which contains CCC supplies. LPA observed no bodies of water on site. A waiver for the use of a staggered outdoor play yard will be needed from the Department prior to Licensure.

Application documentation was reviewed at the CCLD, prior to the inspection. LPA was informed no firearms are on site.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations with Facility Representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The San Luis Obispo County Fire Department granted a Fire Clearance for a total capacity of 32 children on 1/9/23. Based on indoor and outdoor space, toilets and sinks, as well as LPA’s record review, this facility meets Title 22, Division 12 requirements for a CCC with a capacity of 32 children, however a waiver for the staggered use of the outdoor play yard is need. As such a Provisional License is granted given the aforementioned, effective forthwith 1/17/22
