On 10/30/23, Licensing Program Analyst (LPA) Elvin Baddley made unannounced inspection to the aforementioned Child Care Center (CCC) in order to initiate and investigate of the above allegations. LPA met with Merideth Eades, Director of the CCC, and explained the nature of the inspection. LPA notes 13 children are on site at the time of the inspection, along with 3 teacher providing care and supervision.

The investigation included record reviews, interviews of the Director and a sampling of teachers on site. Children in care were not interviewed. The allegations of the complaint referenced the CCC having both pests (ants) and rodents. Further, the complaint referenced the CCC staff members not meeting the qualifications to care and supervise day care children.

With respect to the CCC having pests and rodents, the Director contacted pest and rodent control vendors upon learning of the aforementioned being on site. The vendor was unable to take preventative actions until the children when not on site. LPA notes no pests or rodents were observed on site and LPA

On 10/30/23, Licensing Program Analyst (LPA) Elvin Baddley made unannounced inspection to the aforementioned Child Care Center (CCC) in order to initiate and investigate of the above allegations. LPA met with Merideth Eades, Director of the CCC, and explained the nature of the inspection. LPA notes 13 children are on site at the time of the inspection along with 3 teacher providing care and supervision.

The investigation included record reviews, interviews of the Director and a sampling of teachers on site. Children in care were not interviewed. The allegation of the complaint referenced the CCC operating outside of ratio.

The allegation of the complaint did not reveal corroborating information. In essence, interviews with staff members revealed the CCC operated within ratios and the Director included herself into the facility ratio to ensure children and staff ratios were in line with regulations and staff members had the opportunity to breaks when needed.

observed pest and rodent traps on site inaccessible to children in care. As such the allegations are substantiated, however no deficiency is cited

With respect to the allegation of CCC staff not meeting the qualifications to care and supervise day care children, LPA conducted a recorded review of all staff member and found S1's Mandated Reporter certification had expired on 10/3/23.

Based on LPA's observation and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Health and Safety 1596.8662(b)(1), is being cited on the attached LIC 9099 D.

A closing interview was conducted with the Director. Director was provided and advised of their Right to Appeal (LIC 9058). LPA informed Director of the need to provide a plan of correction to CCLD as well as the time which the plan of correction needs to be submitted to CCLD.

The Notice of Site Visit (LIC 9213) was provided to the Licensee as required by H&S Code Section 1596.817. The Notice of Site Visit must remain posted for 30 days or a civil penalty of $100.00 may apply.
