25 | On 6/8/26, at 9:45 AM, Licensing Program Analyst (LPA) Matthew Sapien conducted an unannounced plan of correction (POC) visit for the abovementioned CCC. This inspection is to follow up on an annual random inspection conducted on 6/2/26. LPA met with Valerie Montano, Licensee of the FCCH, and explained the nature and purpose of the inspection. The LPA, in the company of Licensee, toured the FCCH. At the time of the inspection, LPA observed 13 day care children present, 5 of which were infants. In addition to the Licensee, LPA also observed the Licensee's mother helping with the care and supervision of children (cleared and associated).
During the 6/2/26 visit, LPA issued two Type B deficiencies and one Type A deficiency. In regards to the Type A citation issued, LPA issued this citation due to there been 11 children present at the time of inspection, 5 of whom were infants. In addition to the Type A deficiency, a plan of correction was also given to the facility and was listed as the following:
ยท On or before, 6/3/26, Licensee will submit written plan of correction to the Department on how future instances will be avoided moving forward. LPA provided a copy of capacity regulations for the Licensee to follow. Licensee will also give copies of today's report and LIC 9224 to current parents that have children in care and to future parents over the next 12 months.
During today's inspection, LPA viewed the number of children and staff present. Through interview and observation, LPA again determined the FCCH to not be in compliance with Title 22 regulations like the 6/2/26 visit.
During today's visit, 1 Type A citations was issued (see LIC 809-D page).
Upon receipt, the Licensee shall provide copies of this licensing report to parents and guardians of children in care and to parents and guardians of children newly enrolled at the facility during the next 12 months. (CONT. LIC 809-C, Page 2)
