On 2/11/25 at 11:50 AM, Licensing Program Analysts (LPAs) Matthew Sapien and Gigi Reyes conducted an announced Pre-Licensing Inspection of the abovementioned home and met with Applicant, Rachel Puente. LPAs informed Applicant of the nature and purpose of the inspection. Applicant informed LPAs of the intention to maintain operating hours of the Family Child Care Home (FCCH) from 7:30 AM-11:30 PM, Monday-Friday. Applicant intends to care for children 0 years of age to 13 years of age. Once licensed, applicant also informed LPAs that Applicant will begin childcare services as soon as possible. Applicant was informed that changes in licensing hours or the ages of children supervised and cared for can be altered upon notifying CDSS CCLD of the modifications or changes. LPAs note 1 child in care (Applicant’s daughter) at the time of the inspection. The Applicant’s husband was also present upon arrival of LPAs (cleared and associated).

LPAs, in the company of Applicant, toured the interior and exterior of the home in its entirety. The home is a 3 bedroom and 2 bathroom three story dwelling. The home's living room (converted day care room), one bathroom, dining room, kitchen, backyard deck, and front yard will be used for childcare services, while the remainder of the home will be excluded from children in care. The excluded areas include the master bedroom, master bathroom, 2 guestrooms, backyard area, and garage.

The home has proper spacing and ventilation for children in care. LPAs observed a fireplace in the day care room of the FCCH, which is boarded up by wooden planks. LPA observed age-appropriate toys, furnishings, and equipment throughout the day care room of the FCCH. The day care room is enclosed by child safety gates. The hallway bathroom, to be used by children in care, is observed to be clean and free of toxins. The bathroom cabinets contain child safety latches. Cleaning compounds are kept in the master bedroom, garage, and in the kitchen area in elevated and locked cabinets. Sharps are also located in an elevated kitchen cabinet beyond the reach of children in care. Personal (CONT. LIC 809-C, Page 2)

medication is stored in a master bedroom closet which is an excluded area. LPAs also observed a First Aid Kit in an elevated cabinet above the refrigerator. Importantly to note, each kitchen cabinet that contains potentially hazardous items to children in care are made secure by child safety latches. Moreover, each room that is excluded from children in care is made inaccessible by child safety knobs, locks, and screens. LPAs observed the home to be clean and orderly.

LPAs observed a fire extinguisher (2A10BC) in the home which was purchased on 11/22/24. LPAs reminded Applicant about the responsibility to service or purchase a regulation fire extinguisher annually. The home has both smoke and carbon monoxide detectors throughout. A combination carbon monoxide and smoke detector was tested at 12:26 PM. The detector was found to be operational. The residence has operating telephone services.

The home's backyard is enclosed by wooden fencing throughout. As previously mentioned, the only area accessible in the backyard is a wooden deck that has sufficient fencing. Applicant informed LPAs that this area is solely to be used for eating and for arts and crafts activities. The footing of the backyard area that is not accessible to children in care is varied (artificial turf, dirt, and concrete). The backyard's entry and exit gates are secured. LPAs observed two medium sized dogs in the backyard, both of whom are fully vaccinated. Applicant informed LPAs that the dogs will have interactions with children in care. LPAs note that there were no bodies of water observed on site. Applicant informed LPAs that there are no firearms or ammunition on site.

As previously mentioned, the front yard will be accessible to children in care. The front yard play area is enclosed by metal fencing that is 47 inches throughout. The play area footing is solely artificial turf. LPA also observed one large shed in the play area that currently possesses car tires and is locked. Applicant informed LPA that a regular walk-through of the area will occur before letting children outside for play and that children in care will always be directly supervised when engaged in activities.

LPAs record review revealed Applicant's Mandated Reporter training was completed on 11/9/24 (expiration 11/9/26), and Pediatric CPR/First Aid (EMSA approved) training was completed on 3/11/24 (expiration 3/11/26). Applicant completed their Preventative Health Training on 11/21/24, and their CCLD Orientation on 11/5/24. LPAs reminded Applicant about the obligation to maintain current training and certifications. The Applicant provided proof of control of property (mortgage statement) (CONT. LIC 809-C, Page 3)

LPAs reviewed with Applicant the LIC 311D (Forms/Records To Keep In Your Family Child Care Home), children’s forms/records, facility forms/records, and information to be posted prominently.

The Applicant will be providing Incidental Medical Services (IMS). IMS policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPAs discussed the safe sleep regulations with Applicant and discussed The Child Care Licensing Safe Sleep webpage as additional resource at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep. LPAs also informed the Applicant about the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

On today’s date, 2/11/25, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.


Applicant was also informed of the MyChildCarePlan.org website, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. (CONT. LIC 809-C, Page 3)

At this time, the front yard play area that is enclosed by metal fencing, measured at 47 inches, will be temporarily inaccessible due to the sturdiness, or lack thereof, of the fencing. The front yard play area will be subject to Program Manager for further review.

The application for Small FCCH License is pending until the administrative action against applicant and spouse are cleared and resolved.
