On 06/03/2024 Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit. LPA met with licensee/administrator Ligaya Badilla and explained the purpose of today's visit.

During this visit LPA conducted interviews with staff and clients at random. LPA made observations of the restrooms, observed lunch portions being served during today's visit, facility client menu, and food supply. Bathrooms observed were not seen as uncleaned or soiled as described. Per the licensee, and staff present, the restrooms are cleaned regularly every other day and as needed if there if odors or mess left behind. LPA observed lunch being served and it included vegetables, proteins, and other items. Additional snacks and fruits are left out for client consumption as well that clients are free to use. Food supply is plentiful and in place. Per licensee and staff, if a client wants additional servings, they will supply the client. Clients interviewed today do not indicate inadequate food service in regards to portions or nutritional value. LPA observed facility menu and received copies during the visit. These allegations are unsubstantiated.

Based on these observations, the above allegations are UNSUBSTANTIATED.
Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the above allegations are unsubstantiated at this time. Report is reviewed with licensee and a copy is provided on this day.
