NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On January 9, 2023 at approximately 8:15am, Licensing Program Analyst (LPA) Catrina Quimbo conducted an announced, case management inspection at The Carey School. LPA met with director, Meagan Vincent, and explained the purpose of the visit. Due to an emergency located at facility #414004826, director requested to temporarily operate child care at facility #410517519. Director also requested to increase capacity from 18 preschool children to 38 preschool children. Operations at Facility #414004826 have temporarily ceased due to flooding at facility. Hours of operation will be from Monday to Friday 7:30am to 6:00pm. The Carey School received a fire clearance 11/17/2022. Per director, new fire inspection will be conducted 01/09/2023. Present during LPA's visit included 37 preschool children with 7 staff (including director). The facility was inspected, indoors and outdoors, for health and safety hazards and measured to calculate capacity. Indoors: Preschool program has access to operate out of 4 rooms on The Carey School campus that have been converted to preschool classrooms. The "PK-R" classroom (originally music room) has a total square footage for 802.79 sq ft. allowing for a capacity of 22 children. The "PK-K" classroom (originally multi-purpose) has a total square footage of 876.92 sq ft. allowing for a capacity of 25 children. The "KG" classroom (originally kindergarten classroom) has a total square footage for 774.62 sq ft. allowing for a capacity of 22 children. The Library has a total square footage of 419.53 sq ft. allowing for a capacity of 12 children. Due to the library being ongoing space that is converted into a classroom and library, library is not counted towards overall total square footage. The total allowable capacity equals 2,454.33 sq ft. allowing for a total capacity of 70 children. LPA observed each classroom is equipped with a fire extinguisher, fire sprinkler and smoke alarm system through The Carey School. First Aid kits are available in each classroom. LPA observed electrical outlets in classrooms are made inaccessible by child safety covers. Each classroom is equipped with appropriate furniture and equipment that were observed to be in good working condition. LPA observed a variety of children's toys and equipment that were age appropriate. Poisons, chemicals and other hazardous materials were stored inaccessible in classroom's high shelves. The "KG" and library classrooms include bathrooms. Bathrooms include 2 sinks and 3 toilets. The library classroom includes an additional sink located in the classroom that is accessible to children in care. LPA observed bathrooms to be in proper working order, equipped with appropriate sanitation products. Total allowable sink and toilet ratio located in classrooms equal 45 children.

Preschool classrooms that do not include a bathroom (P-K R and P-K K) plan to use and share The Carey School girls' restroom on site that include 2 sinks and 4 toilets. Per director, classrooms that do not have a bathroom will be walked by staff to and from the restroom. Director is aware children in preschool program and school age program are to use the restroom simultaneously. Total allowable sink capacity in girls' restroom is 45 children. Total allowable toilet capacity in girls' restroom is 60 children.

Accessible sinks do not provide water that is warmer than 120 degrees Fahrenheit. Children will be signed into and out of program through children's authorized representatives' written signatures.

Outdoor: The outdoor area includes a basketball court, soccer field, garden, large sand box play area and large wood chip area. The outdoor area that will be utilized for preschool program includes the garden, soccer field, large sand box play area and large wood chip area only. The outdoor area that will be utilized for preschool program measures for 10, 461.47 sq ft. allowing for a capacity of 139 children.

Entire outdoor area is enclosed with an at least 4 ft. high fence. Outdoor area was observed to be clean. Adequate shade is provided by trees in surrounding areas and designated area with a tarp. Large sand box and large wood chip area include play structures that have resilient padding underneath. Director is preschool children using the play structures must always be supervised. Director is also aware preschool program children and school age children are to use the outdoor area separately. Water is available indoors and outdoors through children's refillable water bottles and accessible water stations.

During LPA's visit, LPA received from director waiver requests to share outdoor area and girls' bathroom with school age children.

No deficiencies were observed today. Emergent operations at Facility #414004826 have been approved to resume at Facility #410517519. LPA advised director to notify department when operations at facility #414004826 resume and/or operates at Facility #410517519 cease.
