25 | On February 27th, 2025 at approximately 9am, Licensing Program Analysts (LPAs) Tapia-Mandujano and Wadhwa arrived at facility for a Case Management-Incident. LPAs met with Director, Lisa Becker and assistant Director, Deanna Castillo. The purpose of the inspection was to discuss the Unusual Incident Report that was reported to the department on February 11th, 2025.
Present in the facility are 85 preschool age children supervised by 9 staff. All staff present during todays inspection have fingerprint clearance and are associated to the facility. Facility operates Monday-Friday from 7:30am-6pm, year round with periodic breaks. Facility is a combination center on the second level with an Infant license on-site (first level). Facility is licensed in 10 classrooms and currently are operating in 7 classrooms.
During today's inspection, LPAs conducted a health and safety inspection indoors and outdoors and interviewed Director and Assistant Director about the incident reported. Based on interviews conducted, the facility was notified by a parent on February 11th, 2025 of the incident that occurred on February 5th, 2025.
Based on interviews that were conducted with S1 (third party agency staff) and T1 (facility staff), C1 (child enrolled) eloped from the classroom line when transitioning from outdoors to indoors. T1 ran after C1 to help C1 come back to the line with the rest of the children. S1 (who was present to support (C1)) observed C1 being grabbed by the wrist by T1. Per Director, S1 notified their agency's director but did not communicate about the incident with the facility.
Per conversation with Director and Assistant Director, once they were notified by the parent, the facility reported internally and externally. The facility conducted an internal investigation. During the investigation, T1 was put on administrative leave. Internal investigation concluded as "unsubstantiated". T1's course of action was a "final coaching" that consisted on a one one meeting with Director and retaking training. Director also took action in communicating with the third party agency to ensure that any incidents that occur at the facility are told to the facility administrators when involving the children and staff.
LPAs have recommended to conduct additional training for all staff of when to report any incidents that occur in a timely manner. LPAs also recommended to have written expectations about reporting incidents with third party agencies to ensure the safety of all children.
No deficiencies were issued today under CCR, Title 22, Div. 12, Chapt. 1.
A Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted and report was reviewed and provided to Director, LIsa Becker.
