On January 18, 2024, at approximately 8.40am, Licensing Program Analysts (LPAs)Tso, arrived at the child care center unannounced to close the complaint investigation into the above allegation and met with the teacher, Marisol. The Director, Ingrid Ho returned to the facility at approximately 9am. Present during the visit were the Director, 7 teachers, who supervise 25 children.

Based on LPAs’ gathered information through observation, interviews and records reviews, the agency has investigated the complaint allegation above. The facility failed to comply with the buildings and grounds regulations. The preponderance of evidence standard has been met. The above allegation is found to be SUBSTANTIATED. Type B violation was issued today, January 18, 2024 in accordance with the California Code of Regulations, Title 22, Division 12 & Chapter 1, citation is being cited on the attached LIC 9099D. The Director was aware of the issue and proactively appointed the contractor in late September and early October 2023 to keep the center free of rodents. The deficiency was cleared at this same visit. A copy of this report and appeal rights were discussed and left with the Director whose signature on this form confirms receipt of these reports. Notice of Site Visit was provided. Notice to remain posted for 30 days.

Based on observation, interview and records review, the licensee did not comply with the section cited above that the videos and pictures demonstrating the dead rodent in the facility, which pose a potential health, safety, or personal rights risk to persons in care.
