25 | On March 19, 2025 @ 11:30 am, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced case management visit. LPA met with Director Karina Garcia and the purpose of the inspection was explained. Present in the facility today was Director, 6 staff and 34 children. LPA inspected classroom for Health and Safety Hazards.
Case Management visit was for an unusual incident report, which was self-reported by facility on 03/04/2025. Incident happened in Ladybirds classroom. Per Director, on 02/26/2025 children were washing hands at the sink. Sink is located next to door on left hand side. As C1 was waiting their turn, C1 put their hand in between the gap in the opened door. Per T1, C2 closed the door and C1’s fingers were smashed when door was closed. C1;s parents were called by Site supervisor and parent came to pick up C1. Per Director, C1 seemed fine, and a band aid was put on C1 finger. Per Director, C1 later expressed C1’s fingers hurt when touched. Parent took C1 for x-rays and it was determined child had fractured finger.
During today’s visit LPA spoke with T1 and C1 regarding incident. LPA observed C1’s fingers bandaged. C1 stated C1 had put her fingers between the open gap of opened door. LPA determined incident was an isolated incident and an accident. Director discussed with staff the importance of not allowing children to close doors or lift stoppers to close doors. Staff also had a discussion with children about safety and not to put fingers between doors and not closing doors themselves.
No deficiencies cited today under California Code of Regulations, Title 22, Division 12.
Notice of site visit shall be posted and shall remain posted for 30 days.
