13 | On January 13th, 2025 at approximately 9:30am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced, complaint investigation and met with licensee, Maria Isabel Andino to deliver the findings of the above allegation. LPA explained the purpose of the visit to licensee. LPA toured the home for health and safety hazards. Present in the facility are licensee and assistant supervising 8 children (2 infants and 6 preschoolers). All adults working or living in the facility have fingerprint clearance and are associated.
Complaint was received by the Department on 10/17/24. During the course of the investigation, interviews were conducted with licensee, parents, assistant, and involved parties. LPA also gathered pertinent documents. The investigation determined there is not enough available information to prove that a daycare child sustained unexplained injuries while in care. Per interviews conducted, any accidents/injuries that happen are communicated with families with a picture of injuries or verbally at pick up.
Although the allegations above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations are UNSUBSTANTIATED.
Exit interview was conducted and report was reviewed with License, Maria Isabel Andino and copy of this report was provided. Report was translated into Spanish by LPA Tapia-Mandujano to licensee.
Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required, will result in an immediate $100 civil penalty. This report is public and can be reviewed.
