On April 17, 2025, Licensing Program Analyst (LPA) Zeynep Basak and Licensing Program Manager (LPM) Daniel Oquendo conducted an unannounced visit to the facility to conclude the complaint received on March 12, 2025. Upon arrival, LPA and LPM met with the licensee, Carrie Russell, and explained the purpose of the visit.
During today’s visit, LPA observed 3 adults and 3 children (2 infants and 1 preschooler) present. LPA verified that all present adults had cleared background checks through the Guardian website.
As part of the complaint investigation conducted on March 18, 2025; April 7, 2025; and April 15, 2025, LPA obtained pertinent documentation, reviewed facility records, and conducted interviews with children, parents, and staff members.
Based on the investigation and the information obtained, the above allegations were determined to be unsubstantiated. Therefore, the complaint is considered closed.
The report was reviewed and signed by the licensee, Carrie Russell. The findings were provided during the visit.
An exit interview was conducted, and a Notice of Site Visit was issued and must remain posted for 30 days.
