On July 1st, 2024, at approximately 12:35 PM, Licensing Program Analyst (LPA), Janet Gil, and Licensing Program Manager (LPM), Garfield Leung conducted an unannounced annual inspection. LPA met with lead teacher, Karla Monzalvo, and explained the purpose of the inspection. At the time of arrival for the inspection, center director was away for a appointment. Lead teacher is designated with facility responsibility. At approximately 1:00 PM, Director Elizabeth Villagomez arrived at the facility.

Present during LPA's visit included 13 enrolled children (all preschool age) and 5 staff members. Facility is operating within capacity limits and ratios during LPA's visit. Hours of operation are Monday to Friday from 8:00 AM to 5:30 PM.

With director and lead teacher, LPA and LPM inspected day care areas inside and outside for health and safety hazards. Program is located in Grace Lutheran Church and School in three classrooms for preschoolers and kindergarteners only. Facility is a multi-level building. Per lead teacher, facility operates from first floor only, and the second floor has a classroom that is rented out for a separate music program. LPA reminded lead teacher to ensure all accessible stair ways are properly barricaded to the children in care.

All three classrooms are equipped with multiple first aid kits, a working telephone, and working carbon monoxide detectors. Entire facility has a smoke, fire alarm system and fully charged fire extinguisher implemented by Grace Lutheran Church and School. LPA and LPM observed all cleaning solutions, poisons and other chemicals to be made inaccessible in a storage closet located outside next to the children's bathrooms. The storage closet was locked and inaccessible to children in care. Storage cubbies for children's belongings are located inside each classroom, labeled with children's individuals names.

LPA and LPM observed classrooms to have age appropriate toys, furniture, and materials that were clean and in working condition. Program uses two school bathrooms located outside the classrooms. LPA observed bathrooms to be in working condition with appropriate sanitation products.

LPA observed electrical outlets in classrooms to be made inaccessible with child safety outlet covers. Children are signed in and out of program with parent's written signatures in a binder located in Room 3. LPA observed all children present to be properly signed into program on the date of the inspection. All licensing documentation are properly posted outside of classrooms and are available for review.

Water is made accessible to children, indoors and outdoors, with children's individual water bottles. LPA observed two play areas utilized for preschool program to be fully enclosed and fenced. LPA observed outdoor area to have age appropriate materials that are in working condition. LPA observed a playground with foam padding underneath and a sand box with proper coverage when not in use. Outdoor area has sufficient shaded area. Facility conducts drills every month during the school year. Last conducted disaster drill was on 06/20/2024.

Program does not provide a food service. Children bring their own food that is stored in children's backpacks or lunch boxes. Per lead teacher, if food brought from home needs to be refrigerated, or heated up they do offer that option to children in care. Classrooms have sufficient amount of sleeping mats that are utilized for nap time. Per lead teacher, sheets and blankets are taken home once a week and washed by children families.

LPA reviewed 6 children's files and 5 staff files. Children's files were complete with a record of emergency identification information on file. Staff files were incomplete. Lead teacher was the only staff with a Mandated Reporter Training Certificate on file. Type B deficiency is being issued on this day in accordance with the California Code of Regulations, Title 22, see LIC 809D. All staff does have a valid CPR/First Aid certificate that will expire 10/2025. All staff do have proof of required immunization that were made available for review.

Director and lead teacher are aware that all staff hired are required to complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com. LPA encourages director to frequently visit our website at www.ccld.ca.gov for licensing regulations and new updates.

Director and lead teacher were informed that as of September 1, 2016, a person may not be employed or volunteer at a child care facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662.

Director and lead teacher were reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
