25 | On October 30th, 2025, at approximately 1:00 PM, Licensing Program Analyst (LPA), Janet Gil, conducted a case management inspection and met with the lead teacher, Liezl Caparas. Director Kelly Wheatley was not present during the inspection. Per lead teacher, director is on leave until the month of January. The purpose of the inspection was explained to the lead teacher. Facility operates as a dual license. Present during today's visit for the preschool license, included 5 staff members (including the lead teacher) and 12 enrolled children. Present for the infant license, including 5 staff members and 6 enrolled children. Facility operates within its capacity and in accordance with the required ratio of staff to children. All adults present working in the facility have present criminal record clearance on file.
This case management inspection was related to the Unusual Incident Report (UIR) submitted by the lead teacher, Liezl Caparas, on October 24th, 2025. LPA discussed the nature of the incident during the inspection. The incident occurred on October 24th, 2025, when a child(C1) fell off play structure, and scraped his nose/upper lip. Per lead teacher, ice was applied to the area by staff after the incident, and parents were notified. Lead teacher stated, C1 returned to day care on October 25th, 2025, and is doing well. C1 was not present during inspection
LPA inspected the facility indoors and outdoors for health and safety hazards, as well as the area where the incident occurred. LPA observed padding under outdoor play structure.
No deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1.
A copy of today’s report was given to the lead teacher, Liezl Caparas.
A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
