LPAs observed the outdoor play area to be free of debris and other loose articles. The play area is enclosed by a fence that is at least four feet high. LPAs observed a play structure slide that was attached to the fence with a bungee cord. A screw was loose, and the slide was detached from the structure. Per D1, the play structure is not in use and is not intended for use by children in care. LPAs did not observe children playing nearby or using the play structure. D1 removed the play structure from the outdoor play area during the visit. LPAs observed that toddlers use an adjacent facility’s (414004933) outdoor play area. LPAs advised that a waiver request shall be submitted that includes a schedule to ensure that children from both facilities are not co-mingling. D1 stated that they agreed. There are no swimming pools or other similar bodies of water present. There are no firearms stored in the facility.
LPAs reviewed five staff files, six children’s files, and facility records. There is at least one staff member present with current Pediatric First Aid/CPR training. All staff members have completed Mandated Reporter Training. Staff qualifications are available for review. All children’s files were observed to be complete. Sleeping logs are maintained for toddlers under the age of 2. All required postings were observed to be posted and available for review immediately upon entry to the facility. Children are signed in and out of the facility via paper sign-in sheets.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.
