25 | On 1/12/2026 at 1:25PM., Licensing Program Analyst (LPA), Luis Gomez met with Licensee, Oksana Myzhala. The purpose of today’s visit was explained and was for an unannounced, annual/ random inspection. Present was the licensee and assistant, caring for 9 children. (4 infant-age, 5 preschool-age). Adults have criminal record clearance on file. The home is a three-bedroom, two-bathroom, two level townhouse.
LPA advised licensee to ensure two adults are present at all times, when operating at large capacity. Advisory Note: Technical Violation (LIC9102TV) was issued. Per licensee, days and hours of operations are: Monday- Friday, 7:30 AM., to 5:30 PM. The areas of the home designated for childcare are: Lower Level: Living Room (Playroom); Dining Room (Activity/ Napping Area); Bathroom #1; and Backyard Area. The areas of the home designated as off-limits are: Upper Level: Bedroom #1; #2; #3; Bathroom #2; and Lower Level: Garage and Kitchen. LPA inspected the home inside and outside for health and safety hazards.
At 1:35PM., the following was observed: Facility clean and neat, with age-appropriate playthings available for the children. The floor and ground surfaces were clear of obstructions or hazards. The furniture, materials, and toys were in good repair, free of sharp corners or splinters. LPA observed one table with several chairs, scaled to the appropriate size. The home's fireplace and staircase leading to the 2nd level have been barricaded.
For napping services/ scheduled rest, LPA observed several infant play pens stored in facility. The play pen were free of loose objects with clean, tight fitted sheets. Per licensee, sheet and beddings are washed weekly onsite.
The facility bathroom (1) was observed clean, with faucet and toilet in operating condition. The facility was comfortable with adequate ventilation and lighting. The facility was equipped with a telephone service; functioning smoke detector; and fire extinguisher (2A:10BC) located in facility. LPA advised licensee to obtain testable carbon monoxide detector.
