On 3/17/2025 at 8:45AM., Licensing Program Analyst (LPA), Luis Gomez met with Assistant Director, Courtney Gromowski. The purpose of today's visit was explained and was for an unannounced, annual/ random inspection. Director, Rowena De Los Angeles arrived during today’s inspection. This facility is licensed to operate a preschool-age program. Present was Director and 11 staff supervising 43 children. Staff presents have criminal record clearances on file. LPA reminded assistant director to ensure all children preset have been signed-in by guardians. Advisory Note: Technical Violation (LIC9102TV) was issued.

The preschool program utilizes five classrooms: Kindergarten Preparation, 4- 5-year-old; Monarch #1, 3 year old; Monarch #2, 3 years old; Butterfly #1, 2 year old; and Butterfly #2, 2 year old, and two, shared outdoor play spaces, preschool (Primary) yard and the ‘2’s (Side) yard. Per director, the Monarch 2’s classroom is not currently in use. Outdoor space waiver waiver for rotational use of outdoor space was observed posted. LPA inspected facility, indoors and outdoors, for health and safety hazards.

At 8:55AM., LPA observed the following: Classrooms was neat, clean, with age-appropriate materials and playthings available for the children. Floors and surfaces were clear of obstructions or hazards leading to the exits. Classrooms were equipped with cabinets, tables, and chairs scaled to the appropriate size. Classrooms have labeled cubbies for belongings. The children’s bathrooms were clean, with fixtures in operating condition, and adequate supplies for handwashing.

For scheduled rest/ nap services, LPA observed stackable plastic cots available. Per director, blankets and supplies are washed weekly by families. Facility has adequate ventilation; lighting; and was a comfortable temperature. Facility had a functioning telephone service and fire extinguishers (2A10BC) and testable carbon monoxide detector. The kitchen/ storage areas were free of any liter/ rubbish, and all food items reviewed were current and had been properly stored.

At 10:00AM, LPA reviewed the outside play spaces, including the preschool yard and side yard. LPA observed absorbent rubber padding /turf installed around climbing structures. Area was completely enclosed with seated available shading available. The climbing structures were observed properly anchored. Sandbox area was free hazardous items. Per director when not in use, tarp used to cover sandboxes.



Per director, water services are provided with refillable bottles brought by guardians. Water bottled are taken to the outdoor spaces for children to drink as they wish.

At 10:30AM., LPA reviewed facility records including a sample of the children files and personnel files. The personnel files reviewed contained Teacher's Proof of Qualifications; Acknowledgement to Report Suspected Child Abuse (LIC9108); Immunization Record; and Personnel Record.

A sample of the children’s files were reviewed and contained the: Identification and Emergency Information (LIC700); Notification of Parent’s Rights (LIC995); Consent for Emergency Medical Treatment (LIC627); Immunization Record; and Personal Rights (LIC613A).

Staff's Cardiopulmonary Resuscitation / First Aid certification was current, expiring on: 4/14/2026.


Facility emergency disaster drill (Fire and Earthquake) was completed on: 1/31/2025 and 10/16/2024. LPA reminded director to ensure drills conducted are logged.

Required postings including the: Facility License; Notification of Parent’s Rights (PUB394); Personal Rights (LIC613A); The Passenger Safety Laws (PUB269); Food Service; Outdoor Space waiver; and Emergency Disaster Plan (LIC610). (REFER TO 809C, FOR CONT.)

Assembly Bill (AB) 2370, chapter 676, statues 2018 requires all licensed childcare centers (CCC’s) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and every five years after the date of the first test. LPA has verified that lead testing procedure was completed in accordance with the Written Directives outlined in PIN 21-21- CCP.

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Based on today's inspection, no deficiencies were cited in the areas evaluated according to the Title 22 Division 12 Chap. 1 Ca. Code of Regulations. An exit interview discussing facility evaluation report was conducted with Director, Rowena De Los Angeles.
