25 | On 5/20/2026 at 12:10PM., Licensing Program Analyst (LPA), Luis Gomez met with Director, Rowena De Los Angeles. The purpose of today's visit was explained and was for an unannounced, annual-random inspection. Present was Director and 10 staff supervising 57 children. The children present had been signed-in by guardians. This facility is a licensed preschool program with infant-age program on-site. The day and hours of operation are Monday- Friday, 8:00AM- 6:00PM. The program utilizes four classrooms: Butterfly 2 (2 year old), Monarch 1 (3 years old), Monarch 2 (3 years old), TK Classroom (4 year old) and two shared outdoor play areas (large and small). Per director, the butterfly 1 classroom is currently closed for today. LPA inspected facility, indoors and outdoors, for health and safety hazards.
At 12:30PM., LPA observed the following: The facility was neat and orderly with age-appropriate equipment available for the children. All floors surfaces, carpets and exit pathways were clear of obstructions or hazards. The accessible furniture and materials inspected were in good repair, and free of sharp corners or splintered edges. The classrooms have labeled cubbies available children’s belongings. The classrooms were equipped with added cabinets for staff belongings. LPA observed several tables/ chairs were scaled to the appropriate size for food services.
The bathroom, located in hallway, was observed clean with adequate supplies for washing hands. The butterfly 2 classroom had a changing table, available for the staff as needed. The accessible trash bin, designated for the disposal of solid waste had been covered. The cleaning detergents, spray bottles, and wipes were made inaccessible and stored on a high-shelf.
The facility had adequate ventilation, lighting, and were maintained at a comfortable temperature. The facility had a functioning telephone service; carbon monoxide detector; smoke detector (built-in); and fire extinguisher (3A40BC) in hallway. (REFER TO 809C, FOR CONT.)
