At 9:30AM., LPA inspected the backyard area. Back yard Area was enclosed with tall fencing, and sharp corners have been covered. Turfing absorbent material had been installed for added safety and plaything were in proper repair.
At 9:50AM., LPA reviewed the facility records including the children’s and staff files. Staff files were reviewed and included the Notice of Employee Rights (LIC9052).
At 10:00AM., Based on Record Review, LPA confirmed staff's proof of mandated reporter training (AB1207) missing from 2 of 3 facility records.
At 10:05AM., Based on Record Review, LPA confirmed staff's proof of immunization and TB clearance missing from facility files. Advisory Note: Technical Violation (LIC9102TV) was issued.
Children’s files were reviewed and included the Identification and Emergency Information (LIC700); Consent for Medical Treatment (LIC627).
At 10:40AM., Based on record review, LPA confirmed child (C1) proof of immunization missing from facility records. Advisory Note: Technical Violation (LIC9102TV) was issued.
At 10:55AM., Based on record review and interview, LPA confirmed licensee not maintaining proof of 15 minute review during infant nap.
LPA reminded facility to conduct and log emergency disaster drills every 6 months.
The required postings are posted by the entry way including the: License and Notification of Parent’s Rights (PUB379).
Per licensee, isolation of an ill child is in the Playroom.
Facility was reminded that all adults 18 years and over living in the home, person who provides care and supervision to children, and staff who have contact with children, including employee and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain criminal clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
