25 | On February 27, 2025, at approximately 1:30 pm, Licensing Program Analyst (LPA) Zeynep Basak conducted an unannounced, plan of correction (POC) visit. LPA met with helper 1 and the licensee, Galina Galant arrived 30 minutes later. LPA observed the licensee was not also at home when LPA arrived for the previous inspection and arrived 15 minutes later. LPA discussed it with the licensee and Technical Assistance was issued today. LPA explained the purpose of the visit to the licensee.
Present during the visit were the licensee, two helpers, and 12 children (3 infants and 9 preschool children). The facility was within the ratio of staff/children capacity.
On February 20, 2025, LPA Zeynep Basak met with the licensee for an annual inspection. During the visit, LPA cited licensee two Type A for an adult (A1) working with children in the facility with no criminal background clearance along with civil penalty and overcapacity. LPA also cited six Type B deficiencies for the following deficiencies.
Missing immunization records for children and helpers
Missing LIC 9227 forms in the infant’s file
Missing CPR and expired Mandated Reporter certificate of the licensee.
Not documenting a 15-minute sleep log
LPA conducted a POC visit on February 27, 2025, to clear type A and B deficiencies. During the POC visit, LPA observed a person with no criminal background staying in an off-limit area and not associated with children since he had not received a clearance letter yet and his status is still in process on the Guardian website. LPA discussed it with the licensee and the licensee was aware the adult cannot be in the daycare and associate with children in care.
