On 8/9/2023 at 9:10AM., Licensing Program Analyst (LPA), Luis J. Gomez met with Director, Laura Doctor. Purpose of inspection was explained and was for an Unannounced, Annual Random inspection. Facility is a licensed preschool-age center. Present was the director and 5 staff supervising 30 children. Staff members present have criminal record clearances on file. Day and hours of operation are: Mon- Fri 7:45AM- 5:15PM. Program operates year-round. Program utilizes three classrooms: Di Vinci, 3 year olds ;Van Gogh, 2.5- 3 years old ;and Silverstein, 4-5 years old, and two, outdoor play areas: Patio and 211 Playground. LPA inspected facility, indoors and outdoors, with director for health and safety hazards.

At 9:30AM., LPA observed the following: Classrooms was clean, orderly, with age-appropriate playthings available for the children. Floor/ground surfaces were free of any obstructions or potential hazards. LPA observed labeled cubbies for storage of children's belongings. Accessible furniture, books, and blocks inspected were free of sharp corners or splinters. Classrooms are equipped with child- sized tables and chairs for snack and seated activities. Children’s bathrooms are maintained clean with fixtures in operating condition. The staff bathroom is located separately. Classrooms have adequate ventilation and lighting.

At 9:35AM., Based on observations, LPA confirmed several disinfectant wipes dispensers stored accessible to day-care children. Director removed dispensers during inspection.

Outlets and trash bins have been covered. Classrooms has built-in smoke detectors, functioning carbon monoxide detector, telephone service, and several fully charged fire extinguishers: 2A:10BC. First aid kit was reviewed and was fully stocked.

At 10:10AM, LPA inspected the outdoor play areas. Play area was completely enclosed, with sufficient shading. Rubber padding has been installed around and underneath play structure for added safety. For water services, fountains are accessible to children.

At 10:20AM., Based on observations, LPA confirmed several children tricycle with peeled paint, in outdoor play area (211 playground). Advisory Note: Technical Violation (LIC9102TV) was issued. Tricycles were removed from area during inspection.

At 10:30AM LPA reviewed facility records including 5 children’s files and 5 personnel files. Personnel files were reviewed contained the: Notice of Employee Rights (LIC9052); Education Verification; Proof of Required Immunization; Updated Mandated Reporter Training Certifications (AB1207); and Personnel Record (LIC501).

Children's files were reviewed contained the: Consent for Emergency Medical Treatment (LIC627); Immunization Records, Identification and Emergency Information (LIC700); Personal Rights (LIC613A); and Notification of Parent’s Rights (LIC995).

Staff's ‘Cardiopulmonary Resuscitation and First Aid’ certification, was current, expiring: 04/2025.

Emergency disaster drill are conducted on-site, with the last drill completed on 5/31/2023, and is properly logged.

LPA observed required posting in facility, including the: Facility License; Notice of Parent’s Rights (PUB394); Emergency Disaster Plan (LIC610); Earthquake Preparedness Checklist; Snack Menu (August, 2023); and Child Passenger Safety Laws (PUB369).

LPA inspected kitchen/ food storage area. Food items inspected were stored in cabinets. Area was clean and free of trash or rubbish.

Medication and IMS Plan was reviewed during inspection.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manuel – Regulations Interpretations and Procedures for Child Care Centers Section 101173 and 101226. When an IMS is provided, an updated Plan of Operations that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in Child Care Center. A civil penalty of $100.00 minimum/ day up to $500.00 maximum per day/per person will be assessment if this regulation is violated. (REFER TO 809C, FOR CONT.)

Based on today's inspection, deficiencies were observed in the areas evaluated according to the Title 22 Division 12 Chap. 1 Ca. Code of Regulations and cited on 809D.
