The outside play area was reviewed during inspection. The outside area was completely enclosed with hazardous plants properly barricaded. The outdoor playthings were inspected were in good repair. An artificial turf/ grassy area had been installed. The area does not accessible to pools, fishponds, bodies of water or other hazards.
During inspection, LPA reviewed facility records including 6 children’s files and the personnel files. The children’s files were reviewed and included the: Identification and Emergency Information (LIC700); Notification of Parent Rights (LIC995A); and Immunization records.
The personnel files were reviewed and included the: Acknowledgement to report suspected child abuse (LIC9108); and Notice of Employee Rights (LIC9052).
LPA reminded licensee to ensure staff's proof of immunization are stored in the facility records. Advisory Note: technical violation (LIC9102TV) was issued.
At 9:55AM., Based on record review, LPA confirmed licensee's cardiopulmonary resuscitation certification (CPR) / 1st aid certification had expired. Advisory Note: technical violation (LIC9102TV) was issued.
At 10:00AM., Based on record review, LPA confirmed licensee mandated reporter training course (AB1207) had expired. Advisory note: technical violation (LIC9102TV) was issued.
At 10:35AM., Based on record review, LPA confirming licensee is not documenting 15- minute visual review of napping infant-age child in care.
Per licensee, facility conducts required emergency disaster drill (fire and earthquake drill) every 6 months, with last drill logged for: 9/23/2026.
The required forms are posted in entry and include Facility License Certificate and Notification of Parent’s Rights (PUB379). Per license, isolation of an ill child is in the playroom. Per licensee, the childcare service provides children with daily snack, breakfast, and lunch. (REFER TO 809C, FOR CONT.)
