25 | On May 31st, 2024 at approximately 2pm, Licensing Program Analysts (LPAs) Tapia-Mandujano and Alvarado conducted an unannounced inspection Case Management-Deficiencies in concurrent with a Complaint Investigation. LPAs met with Lead Teacher, Ying Zeng and explained the purpose of the visit. Present during inspection was 6 staff supervising a total of 35 preschool age children. Facility is operating within teacher child ratio at this day. All staff present at this time have fingerprint clearance and are associated.
On April 22nd, 2024, LPA Tapia-Mandujano interviewed staff. During the interviews, staff stated how many units they have or are enrolled in, as well if they are left alone with children. LPA Tapia-Mandujano and Alvarado inspected staff files to verify teacher's educational qualifications. Based on file reviews during today's inspection, it was determined that S1, S4, and S5 do not have proper documentation of completed or enrolled in appropriate Early Childhood Education Classes on file.
On April 22nd, 2024, LPA Tapia-Mandujano observed S1, S4, and S5 alone either in the classroom or in the play yard without a fully qualified lead teacher present. During interviews, S1, S2, S3, S4, and S5 stated that at one point S1, S4, and S5 may be alone with children during outdoor play or while covering breaks.
Based on information obtained and observations made on April 22nd, 2024, teacher aides are being left alone without a fully qualified staff present, which poses a potential health and safety hazard. Type B deficiency will be issued.
Facility shall post the Notice of Site Visit for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. This report is public and can be reviewed.
