25 | On December 18th, 2023, at approximately 11:40am, Licensing Program Analyst, (LPA) Tapia-Mandujano conducted a concurrent inspection along with a complaint investigation and met with Licensee, Marilu V. Hernandez. Present in the facility are licensee and assistant supervising a total of 13 children (4 infants and 9 preschool age children). Facility is not operating within capacity limits. All adults present in the facility are fingerprint cleared and associated.
During the health and safety check, LPA observed that there were 4 infants and 9 preschool age children present in the facility. LPA reviewed childrens' files to ensure that ages of the children present. Refer to LIC 809D for deficiency information. LPA explained to Licensee, the capacity limitations.
LPA informed licensee Marilu V. Hernandez that this report dated 12/18/23 documents a Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA informed the licensee Marilu V. Hernandez to provide a copy of this licensing report dated 12/18/23 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days.
