13 | On 3/22/2024, at approximately 8:30AM, Licensing Program Analyst (LPA) Jonathan Tse conducted an unannounced complaint investigation visit. LPA was granted entry to the facility by Licensee, Tatiana Lozinscaia (L1). Present during the visit was L1, a helper (H1), H1's minor daughter, two infants and five preschool age children.
During the course of the investigation, LPA conducted record review, site observations, and staff interviews. The provider denied the allegations. LPA observed the provider to be present upon entry to the facility. Based on interviews with relevant parties, review of relevant documentation, and LPA's observations, there is no direct evidence to prove that the allegation above did or did not occur.
Based on relevant information reviewed, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated at this time.
Appeal rights were provided to Licensee. A Notice of Site Visit was provided and must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with Licensee, Tatiana Lozinscaia. |
