| On 6/26/2024, Licensing Program Analyst (LPA) Hanson Leong conducted an unannounced Plan of Correction (POC) visit to the Child Care Center (CCC) listed above. The purpose of the visit was to follow up with deficiencies that were cited on 4/16/2024. LPA met with the Director, Leny Hernandez, and explained the purpose of the visit. Twenty five children (18 pre-k and 7 toddlers) and five staff members were present during the visit. The five staff members present at the CCC received criminal record clearance from the department. The CCC is within its capacity and in accordance with the required ratio of staff to children.
LPA reviewed staff immunization records and found that all staff members have complete immunization records.
LPA reviewed staff qualifications and confirmed that all staff members have the necessary Early Childhood Education (ECE) units to be classified as aides and teachers.
LPA reviewed the outdoor schedule and found separate times for toddlers and preschoolers.
LPA reviewed the sleep logs for children under the age of two and found them to be properly recorded.
LPA reviewed five random LIC 9224 forms, which are required for receiving a Type A violation (cited on 4/16/2024) and found that they had parent signatures on them.
LPA collected the new director packet from the facility and confirmed that the director's packet is complete, and Leny Hernandez will be added to the system as the director.
The following deficiencies, HSC 1596.7995(a)(1), CCR 01216.1(c)(1), CCR 101216.3(b)(1), CCR 101216.4(a)(2), and CCR 101429(a)(2)(C)(3), that were cited on 4/16/2024 will be cleared during today’s visit.
A copy of today’s report was given to the Director, Leny Hernandez. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Leny Hernandez.
