25 | On May 29, 2026, at approximately 1:15pm, Licensing Program Analysts (LPAs) Tapia-Mandujano and Quimbo conducted a case management-deficiencies inspection. LPA met with Site Director, Chen “Zoe” Liang and explained the purpose of the inspection.
Facility is a combination center with an infant and preschool program with a toddler component in the preschool program. Present during inspection were Site Director and three staff supervising a total of 20 children (8 toddlers and 12 preschoolers).
During inspection, LPAs verified that staff who were present. LPAs observed that there is a staff member (S1) present who is not cleared on the guardian roster. Type A deficiency was given.
LPA Tapia-Mandujano informed facility representative, Chen “Zoe” Liang that this report dated 5/29/26 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Tapia-Mandujano informed facility representative, Chen “Zoe” Liang to provide a copy of this licensing report dated 5/29/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Deficiencies were cited today under CCR, Title 22, Div. 12, Chapt. 1. Refer to LIC 809D for more details.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Site Director, Chen “Zoe” Liang.
