Licensing Program Analysts (LPAs) Andrea Medlin and Jonathan Tse met with facility representative for this plan of correction visit established on 7/5/2023. All deficiencies were due for correction on 7/26/2023 and there was no extension. The following deficiencies previously cited are still not corrected and are being re-cited today:
  • Section 102416(c) - First Aid and CPR certification - This requirement is not met as evidenced by: based on review of Pediatric First Aid and CPR it was found to be expired in 9/2022. It is noted that Licensee showed proof of enrollment in a course on 9/29/2023.
  • Health and Safety Code 1596.8662(b)(1) - This requirement is not met as evidenced by: On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Licensee and helper do not have the required training.\

The following deficiencies previously cited are corrected or cleared today:
  • Section 102417 - Children's files - This requirement is not met as evidenced by: all children's files are incomplete. See LIC 857 - Children's Record Review for a list of what is missing in each child's file. This deficiency is cleared as the children previously enrolled are no longer here.
  • Section 102417 - Fire Extinguisher - Licensee has purchased the correct size fire extinguisher 210AA.


The deficiencies cited on the following pages are in violation of the California Code of Regulations, Title 22, Division 12, Chapter 1. This report is reviewed with facility representative and a copy of this report must be made available for public review upon request. Notice of Site Visit posted and shall remain posted for 30 days.
