25 | On September 18th,2024 Licensing Program Analysts (LPA) Kassandra Medrano and Diana Alvarado met with Berta Garcia Lopez, Facility representative, for an unannounced required –annual inspection. LPA toured the indoor and outdoor areas of the facility during today's inspection. LPA observed the required posted materials, including the Facility License, Emergency Disaster Plan (LIC 610), Earthquake Preparedness Checklist (LIC 9148), Parents' Rights Poster (PUB 393), Personal Rights (LIC 613A), Child Car Seat Law (PUB 269), menus, and Activity Schedule.
Days and hours of operation are Monday through Friday: 7:30am-5:30pm. The Infant/toddler program is Licensed in Rooms: Khit, Wobbly Walkers, Talking Toddlers. Facility is located on campus of Ravenswood Elementary School. On site the licensee has another license, All Five @ Belle Haven (PS), 414004132, which is licensed in Rooms: Este, Centro, Oeste.
The following was observed today: LPA reviewed ten children's and 9 staff files during today's inspection. During review, it was observed that 2 of staff are missing documentation of immunization's. All staff have current certificates of completion of the Mandated Reporter Training for Child Care Workers on file. Based on review of teachers files it was found that 5/9 staff files staff were missing required units. Last fire/disaster drill was completed on 8/6/2024. The staff files that were reviewed have current CPR and First Aid certifications on file. Director understands that there shall be at least one person, with valid CPR and First Aid certifications, on site at all times or present during off-site activities (field trips). During review of children’s files it was observed that infants files were missing complete lic 9227's. Director understands the conditions, limitations, and capacity specifications of the facility license. Director understands that children shall be always visually supervised. Any child(ren) who exhibit symptoms of illness including, but not limited to, fever or vomiting, are not accepted in care. Any child(ren) who become ill during the day, shall be isolated in director's office area.
