Licensing Program Analyst (LPA) Glenn Schnell met with applicants Yan Yu Zhao and Xu Ming Chen today for a pre-licensing inspection. This inspection included a technical assistance inspection for COVID-19 guidance and support. The applicant is requesting 19 infants (ages birth to two years old), with a toddler option component (ages 18 months to 36 months). The infant program will be part of a combination center as the applicant has a licensed preschool for 30 children (ages two years old to entry into first grade) at this same location. The facility plans to operate Monday - Friday; 7:00 AM to 6:00 PM. The facility is located at New Life Community Baptist Church where space is leased. The designated director/site supervisor for the facility will be Wendi Zhang. The facility was inspected today, indoor and outdoor, for health and safety hazards and measured to calculate capacity.

Indoor Infants: There is one room that will be used for the infant program. The toddler option room is in a physically separate space from the enclosed infant room. The toddler room by itself has enough square footage for 12 children. The following square footage was calculated for the infant room: 263.25 sq ft. divided by 35 equals 7 infants. The following square footage was calculated for the space to be used as the toddler option component: 959.748 sq. ft. divided by 35 equals 27 toddler option children. Since the maximum group size for toddler option is 12, the allowable capacity will be 12 toddler option children. Total useable indoor square footage for the infant program (both rooms) is 1222.998 sq ft divided by 35 sq ft equals 34 infants. The indoor spaces are observed today to be clean and equipped with fire extinguisher, smoke and carbon monoxide detectors, cubbies for infants personal belongings, a crib area with three cribs, four cots/mats for older infants, a changing table within arms reach of a sink, and age appropriate toys and equipment for infants. There is storage for infants personal belongings, bedding and other items provided by the parents for the care of individual infants. Parents will provide disposable diapers and wipes for their infants. The office has microwave and refrigerator. Food is provided by the parents, except when an infant transitions to table foods, the program provides two snacks. Food for each infant is stored in a refrigerator in the office. Foods will be properly labeled with the child's name and date. The Church has a kitchen accessible for the staff to use if needed.

Applicant was reminded that cleaning products and other chemicals are not to be stored with foods. During COVID-19, the facility will use any disposable plates, cups and utensils when necessary. Parents will provide bottles/nipples and any other food service items for their own individual infants and the staff will ensure these items are sent home with the infants each day. Napping equipment for older infant children developmentally too old for a crib will be a cot/mat with sheets and blankets will be provided by parents and the napping equipment and bedding will be laundered daily by the parents. Since there are only 3 cribs, the program will be limited to caring for no more than three children at a time who require the use of a crib. Drinking water for the children will be provided by plastic baby water bottles or cups with lids provided by the parents. Medications for infants will be stored in the classroom in high cabinets or in locked boxes in the refrigerators. The foods/bottles in the classroom must be kept completely separate from the diaper changing/sink area of the classroom. All furnishing and equipment is observed to be safe and in good condition. There are garbage cans with tight fitting lids. The indoor space is equipped first aid and emergency supplies. The sink in the infant classroom for children's usage delivers water no warmer than 120 degrees. For toileting infants who are developmentally ready, the staff will use potty chairs. There is a separate bathroom for staff. The isolation room for ill infant will be in the office.

Outdoor Infants: The outdoor area for Infants measures 449.757 square feet allowing for 6 infants. Applicant will ensure all infants are provided with outdoor time each day. Applicant will need to submit a request for a rotation waiver with a rotation schedule since the outdoor activity space can only accommodate 6 children. The outdoor area surface and equipment is observed to be clean, in good condition, and age appropriate. There are no climbing structures. The facility will use a portable shade structure to provide shade for the children. This still needs to be obtained and installed. Drinking water will be made accessible to children by water bottles provided by the parents.

The toddler option room needs to be made physically separate from the preschool program that occupies the other side of the large indoor space. Applicants plan to put up a gate that opens and closes to completely separate both programs. The toddler option program will share the bathroom with the preschool children at separate times. The room to be used was observed to have no health and safety hazards, but needs to be completely set up with toys and equipment for children to use. Applicant/Licensee understands that the age range for the toddler option is 18 months to 3 years old and that authorized representatives must give written permission for their child to participate in the toddler option component as well as for their child(ren) to move out of the toddler option room before the age of 3. Applicant/licensee understands that the toddler option children must be kept physically separate from the preschool age children. Ratio and staffing for the toddler option were discussed.

The following items were reviewed as part of today's visit: Record keeping for staff, children and facility records, maintaining and updating needs and service plans for infants, Care and Supervision of the Children, Child Discipline Procedures- talking and redirection will be used, Emergency Evacuation Procedures, Medication Policies, Isolation of Sick Children, Napping Requirements (Safe Sleep and SIDS discussed), Completion of Individual Infant Sleeping Plans and supervision of napping infants, Food Service for Infants, Transportation-none provided, Parents Rights, and Reporting Requirements. Applicant will ensure all require postings are displayed and visible to children’s authorized representatives at all times. The requirement for the testing of lead in the water was discussed (H&S Code 1597.16). LPA informed the director/licensee to review Provider Information Notice 21-21-CCP (dated 7/28/21) for directives and regulations regarding obtaining a test of the water for lead, and how that information is to be documented and reported to Community Care Licensing. This requirement for testing must be met by January 2023.
The facility will conduct monthly disaster drills and a log will be maintained for review upon request.
The fire inspection was conducted, and approval was received on 11/8/21. An amended fire safety inspection will be requested since the capacity for the toddler option changed from 11 to 12 children.

This facility will provide Incidental Medical Services – IMS. LPA reviewed storage area for medication and equipment/supplies, and reviewed forms that will be used. LPA Schnell reviewed the COVID-19 self assessment guide and required postings with the applicant today. Applicant was informed that access to available Personal Protective Equipment (PPE) may be available through the local child care resource and referral agency.
An electronic copy of this report will be emailed to applicant Yan Yu Zhao on 12/4/21 at email yanzhao129@yahoo.com. Applicant was asked to review the report and return a signed copy of the report to LPA Schnell.
LPA Schnell will recommend licensure of this facility for a capacity of 19 infants, with no more than 12 toddler option aged children. (7 infants/12 toddler option age children) once the following is received:
-Outdoor Rotation Waiver request and schedule is received and approved
-Toddler Option Space is made completely physically separate from the licensed preschool space
-Crib area is arranged to allow for adequate room to place three cribs and four napping mats/cots
-Shade for outdoor space is installed
