On March 5, 2025, at 10:00AM, informal office meeting was conducted at the San Bruno Regional Office. Licensing Program Managers (LPM’s) Leung, Zebila, and Licensing Program Analyst’s Tso, Naves, Alvarado, and Wadhwa met with Licensees Xu Ming Chen and Yan Yu Zhao for an Informal Meeting.

During the meeting, the following was reviewed and discussed:

· On January 23, 2025, during an unannounced annual random Inspection Facility Director Jelena Lee was not present. At 9:15 Facility owner Yan Yu Zhao arrived at the center then at 9:50 Xu Ming Chen arrived at the inspection. Both are owners of the Facility.
o During the inspection two Technical Violation (TV) and two Technical Assistance (TA) were issued regarding staff-infant ratio, Infant Care Center Sleeping Equipment, and Toddler Component in an Infant Care Center.
o A citation was also issued regarding the limitations on capacity of their outdoor waiver.
· On May 10, 2021, both licensees Xu Ming Chen and Yan Yu Zhao signed and dated written plans as to how they will handle emergencies and priorities that will come up with the operation of the Child Care Center as both licensees also operate their own Family Child Care Home.

The Licensee was reminded of the following:

· The facility center needs to have a fully qualified Director present at the facility. If Director is not present, facility needs to have the next designation of facility responsibility.
· Technical Violations can become citations if not addressed.
· Facility must follow their outdoor rotation waiver.

The licensees must understand the importance of adhering to all licensing regulations. Additional inspections will be conducted by the Regional Office.

An exit interview was conducted, and the report was reviewed with the licensee’s, Xu Ming Chen, and Yan Yu Zhao.
