25 | On July 25, 2023 at 1:00pm, Licensing Program Managers (LPMs), Daniel Oquendo, Marie Rodriguez, and Licensing Program Analyst (LPA), Catrina Quimbo, met with licensee, Linda Tong, and assistant director, Bonnie Li, for an informal office meeting.
The purpose of the informal meeting was to discuss the civil penalties and citations issued to facility.
Civil penalties citations discussed during this meeting included:
- Lack of supervision. Facility was cited 04/27/2023 and 06/15/2023 for children being left unattended in a classroom, without any staff supervision.
- Staff Qualifications and Teacher/Child Ratio. Facility was cited 04/14/2023 and 06/15/2023 for unqualified staff providing supervision to enrolled children, without the direct supervision of qualified staff.
- Posting licensing documentation. Facility was cited 04/27/2023 for not posting previous reports with citations, nor providing enrolled children's families the acknowledgement of receipt of licensing reports (LIC9224).
Licensee was reminded:
- Children must be supervised at all times. No child while present in the facility shall be left unattended by a staff member.
- Staff must be fully qualified to count towards teacher/child ratio. Staff who are not fully qualified must not be left alone with children.
- To provide copies of the Evaluation Report that documents any Type "A" deficiencies to parents/guardian of children currently enrolled in care and to any newly enrolled parents/guardians for 12 months from the date of the report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child’s file for verification.
During meeting, LPMs provided licensee with:
- Regulations for staff qualifications, including director, teacher and aides.
- Capacity regulations and ratio for a preschool, toddler option and infant child care center.
- Health and Safety regulation of the posting of licensing reports.
To correct civil penalties and citations issued, licensee plans:
- To maintain communication with all staff and enrolled children's families.
- For each classroom to have fully qualified staff during all hours of operation.
- For staff to complete their required ECE units to be qualified teachers.
