13 | On August 3rd, 2023 at approximately 1:10pm, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced, complaint investigation. LPA was granted entrance by assistant, Jose Pablo Diaz Lopez. Licensee arrived approximately 5 minutes later. LPA explained the puprpose of inspection with Licensee, Maria Isabel Vazquez and explained the above allegations. LPA and licensee toured the facility for health and safety hazards. LPA also obtained an updated Children's roster copy.
Complaint was received by the Department on 05/11/23. Present in the facility are licensee and licensee's assistant caring for 4 children (3 infants and 1 preschool age). All adults present in the facility have fingerprint clearance and are associated with the facility.
During the course of the investigation, LPA conducted interviews with involved parties and received pertinent documentation. LPA did not have enough information to determined that licensee hit daycare children and/or that licensee handles daycare children in a rough manner.
Although the allegation above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the above allegations are UNSUBSTANTIATED.
Upon receipt of this report, Licensee shall post the Notice of Site Visit. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain postings as required, will result in an immediate $100 civil penalty. This report is public and can be reviewed.
After today’s visit, an exit interview was conducted, report was reviewed and copy was provided to licensee, Maria Isabel Vazquez. |
