Based on review of the facility's parent and staff handbooks, there is a behavior management plan that is established when a child has repeated behaviors such as biting or hitting. Based on interview with relevant parties, LPAs observed that the facility did follow this procedure and had set up meetings to establish these plans when needed. LPAs were able to confirm that these meetings were communicated with relevant authorized representatives or parents. However, the behavior management plans were not documented and the contents of these plans were unable to be verified.
Based on record review, LPAs verified the qualifications of each teacher or aide. Based on site observations, LPAs did not observe an unqualified staff member supervising children by themselves. LPAs observed that aides were not left alone with children, and were under the supervision of a teacher while providing care and supervision to children.
No direct evidence to support the allegations was observed during the course of the investigation. The facility denied the allegations.
Based on relevant information reviewed, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated at this time.
No deficiencies were cited during today's visit on July 2, 2026. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Yun "Mia" Li. |
