On October 30, 2023 at approximately 8:30am, Licensing Program Analyst (LPA) Catrina Quimbo, conducted a scheduled pre-licensing visit. LPA met with applicant, Grace Yee, and director, Sally Chang, and explained the purpose of the visit.

Department received application September 18, 2023. Fire inspection was also being conducted by fire department during LPA's visit. The applicant is requesting a combination center of a preschool program (2 years to entry into first grade) and infant program (12 months to 24 months) with a toddler option component (18 months to 36 months). Applicant is requesting for 20 infants and 25 toddler option children. Program operates at Island United Church in classroom #1, classroom #2, playground #1 and playground #2 only.

Applicant was reminded toddler option component cannot exceed 12 children. Applicant was reminded infant enrolled children, preschool enrolled children and toddler option enrolled children must be kept physically separate and cannot co-mingle. Applicant was also reminded toddler option children must have written permission/consent from their authorized representatives to participate in toddler option component. Applicant was also reminded infant children must have an infant needs and services plan that must be updated with infants' families.

The infant program will operate Monday through Friday from 8:00am to 6:00pm. The designated director will be Sally Chang. Facility includes 2 classrooms, program office, kitchen, adult restroom and children's restroom. The facility was inspected, indoors and outdoors, for health and safety hazards and measured to calculate capacity. Children will be signed into and out of program by electronic signatures through an electronic application. Applicant was reminded sign in/out sheets must be made readily available for review.

Indoors: The infant program with toddler option component will operate in classroom #2. Applicant has installed a barrier in the classroom to separate the infant enrolled children and the toddler option enrolled children. In classroom #2, the infant area (back portion, located by cabinets) was measured for 338.9 sq. ft. allowing for a capacity of 9 children. The toddler option area (front portion, located towards the bathrooms) was measured for 430.4 sq. ft. allowing for a capacity of 12 children. Total allowable indoor square footage equals 769.3 sq. ft. allowing for a capacity of 21 children.

LPA observed classroom to be well ventilated with appropriate temperature. Infant area and toddler option area are equipped with furniture, toys and materials that were observed to be age appropriate. Flooring includes carpeting and rugs that were observed to be in good, clean condition. Storage for children's belongings was observed to be inside the classroom, that will be labeled with children's individual names. Garbage bins were observed to have tight fitting lids.

Applicant plans to store poisons, cleaning supplies and hazardous materials in classrooms' high shelves. Classroom is equipped with a fully charged fire extinguisher and smoke detector in classroom's high ceilings. Per applicant, smoke detector in classroom is a dual smoke and carbon monoxide detector. LPA was unable to test detector during visit. LPA advised applicant to install additional carbon monoxide detector that can be tested during a future visit. Medications with children’s individual names and required documentation will be stored in appropriate classrooms' high shelves.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Program plans to provide napping infants and toddler option children with cots that are stored in the classroom. Children's families are to provide blankets and sheets. Program plans to store children's sheets in children's cubbies. Per applicant, blankets and sheets will be sent home weekly or as needed.

Applicant plans for isolation area to be in the office that will be equipped with a separate cot. Program's discipline policy was discussed. Applicant plans to use positive guidance and communication with children's families. Applicant stated they are aware there can be no corporal punishment.

In classroom #2 there are 2 bathrooms. There is an individual bathroom with 1 toilet and 1 sink that will be for toddler option use only. Additionally, there is another bathroom that includes 3 sinks and 2 toilets in classroom that will be for preschool use only. There is also an additional shared bathroom (located across from program's office) that includes 2 additional toilets and 2 sinks. Applicant plans to utilize one stall in shared bathroom (located across from program office) for the infant children, if needed.

LPA advised applicant to label designated stall in bathroom for infant children. There is an additional sink in the classroom that was observed to be in working condition. For the infant license, there are total of 2 toilets and 3 sinks. Total allowable capacity for sinks is 45 children. Total allowable capacity for toilets is 30 children. There is a diaper changing table in the classroom that is within arm's reach of an adult sink.

Infant program plans to provide a food service that includes breakfast and afternoon snack only. Applicant plans for children's families to provide lunch for their children. Food will be prepared in kitchen that will be for staff use only. Applicant was reminded poisons and cleaning solutions shall not be stored in kitchen. Drinking water will be made available by children's individual water bottles. Children's water bottles can be refilled on site through filtered water pitchers.

Outdoor: The infant outdoor area (playground #2) is located adjacent to classroom #2. Outdoor area is entirely fenced and enclosed, separate from the preschool outdoor area (playground #1). Total outdoor area measures for 1,420 sq. ft. allowing for a capacity of 18 children. Applicant plans to submit a waiver, requesting to utilize playground #2 as toddler option playground when not in use by infant children. Outdoor area is equipped with toys that were observed to be in good condition. There is a play structure that was age appropriate and has resilient padding underneath (installed under artificial turf). Applicant is aware children must always be supervised going to and from the outdoor area. There is appropriate shade for the infant playground. LPA observed a water fountain in outdoor area. Per applicant, fountain will not be utilized for children. LPA did not observe any bodies of water on site.

The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Applicant was reminded all child care centers operating in a building constructed before 2010 must test their water for lead contamination.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA reminded applicant emergency disaster drills must be documented when conducted. LPA reviewed with applicant and director, the LIC311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education
website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

During visit, LPA was provided copies of updated LIC308, LIC404, and updated LIC401 from applicant.

Based on indoor square footage, maximum number infant program can be licensed for is 21 children; toddler option component not to exceed 12 children.

Prior to recommended licensure, following must be completed:
-Fire clearance
-Posting of all required licensing documentation (Notification of Parents' Rights, Child Passenger Restraint Poster)
-Installation of working carbon monoxide detector.
-Removal of bricks on playground #2.
-Sound alarm on exit fence in playground #2.
-Additional padding installed on corner of cabinet shelves.
-Labeling of bathroom stall for infant children in shared bathroom.
-Request and approval of outdoor waiver.
-Submission of all documents indicated on LIC184C.
-LPA's review and approval of resubmitted documents indicated on LIC184C.
-Management approval (toddler option separation).

Exit interview conducted and report was reviewed with the applicant, Grace Yee.
