On October 30, 2023 at approximately 8:30am, Licensing Program Analyst (LPA) Catrina Quimbo, conducted a scheduled pre-licensing visit. LPA met with applicant, Grace Yee, and director, Sally Chang, and explained the purpose of the visit.

Department received application October 6, 2023. Fire inspection was also being conducted by fire department during LPA's visit. The applicant is requesting a combination center of a preschool program (2 years to entry into first grade) and infant program (12 months to 24 months) with a toddler option component (18 months to 36 months). Applicant is requesting for 24 preschool children. Program operates at Island United Church in classroom #1, classroom #2, playground #1 and playground #2 only.

Applicant was reminded infant enrolled children, preschool enrolled children and toddler option enrolled children must be kept physically separate and cannot co-mingle.

The preschool program will operate Monday through Friday from 8:00am to 6:00pm. The designated director will be Sally Chang. Facility includes 2 classrooms, program office, kitchen, adult restroom and children's restroom. The facility was inspected, indoors and outdoors, for health and safety hazards and measured to calculate capacity. Children will be signed into and out of program by electronic signatures through an electronic application. Applicant was reminded sign in/out sheets must be made readily available for review.

Indoors: The preschool program will operate in classroom #1. Preschool classroom was measured for 856.65 sq. ft. Total allowable indoor square footage equals 856.65 sq. ft. allowing for a capacity for 24 preschool children. LPA observed classroom to be well ventilated with appropriate temperature. Classroom includes furniture, toys and materials that were observed to be age appropriate, and in good condition. There is a carpet in the classroom that was observed to be new and clean. Storage for children's belongings was observed to be inside the classroom, that will be labeled with children's individual names. Garbage bins were observed to have tight fitting lids. Classroom also includes 2 doors to church. Applicant was reminded children cannot go to church area and program will be licensed for classroom #1 only.

Applicant plans to store poisons, cleaning supplies and hazardous materials in classrooms' high shelves. Classroom is equipped with a fully charged fire extinguisher and smoke detector in classroom's high ceilings. Per applicant, smoke detector in classroom is a dual smoke and carbon monoxide detector. LPA was unable to test detector during visit. LPA advised applicant to install additional carbon monoxide detector that can be tested during a future a visit. Medications with children’s individual names and required documentation will be stored in appropriate classrooms' high shelves.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Program plans to provide napping preschool children with cots that are stored in the classroom. Children's families are to provide blankets and sheets. Program plans to store children's sheets in children's cubbies. Per applicant, blankets and sheets will be sent home weekly or as needed.

Applicant plans for isolation area to be in the office that will be equipped with a separate cot. Program's discipline policy was discussed. Applicant plans to use positive guidance and communication with children's families. Applicant stated they are aware there can be no corporal punishment.

In the infant classroom (classroom #2) there are 2 bathrooms. There is an individual bathroom with 1 toilet and 1 sink for toddler option use only. Additionally, there is another shared bathroom that includes 3 sinks and 2 toilets. For the preschool license, there are 2 toilets and 3 sinks. Total allowable capacity for sinks is 45 children. Total allowable capacity for toilets is 30 children.

Preschool program plans to provide a food services that includes breakfast, lunch and afternoon snack. Food will be prepared in kitchen that will be for staff use only. Applicant was reminded poisons and cleaning solutions shall not be stored in kitchen. Drinking water will be made available by children's individual water bottles. Children's water bottles can be refilled on site through filtered water pitchers.

Outdoor: The preschool outdoor area (playground #1) is located right outside the preschool classroom #1. Outdoor area is entirely fenced and enclosed, separate from the infant outdoor area (playground #2). Total outdoor area measures for 2,167.01 sq. ft. allowing for a capacity of 28 children. Outdoor area is equipped with toys that were observed to be in good condition. There is a play structure that was age appropriate and has resilient padding underneath (installed under artificial turf). Applicant is aware children must always be supervised going to and from the outdoor area. LPA did not observe any bodies of water on site.

The requirement for Lead Water Testing was discussed (H&S Code 1597.16). Applicant was reminded all child care centers operating in a building constructed before 2010 must test their water for lead contamination.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA reminded applicant emergency disaster drills must be documented when conducted. LPA reviewed with applicant and director, the LIC311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education
website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. During visit, LPA was provided copies of updated LIC308, LIC404, and updated LIC401 from applicant.

Based on indoor square footage, maximum number preschool program can be licensed for is 24 children.

Prior to recommended licensure, following must be completed:
-Fire clearance
-Posting of all required licensing documentation (Notification of Parents' Rights, Child Passenger Restraint Poster)
-Installation of working carbon monoxide detector.
-Sound alarm on exit fence in playground #1.
-Sound alarm on both doors to church in classroom #1
-Additional padding installed on corner of cabinet shelves.
-Submission of all documents indicated on LIC184C.
-LPA's review and approval of resubmitted documents indicated on LIC184C.

Exit interview conducted and report was reviewed with the applicant, Grace Yee.
