On 1/31/2025 at approximately 8:15 a.m., LPA was continued to annual inspection. During the continuation inspection, the only adult was the licensee and 5 children (preschoolers).

On January 29, 2025, at approximately 2:00 p.m., LPA Zeynep Basak arrived at the facility to conduct an unannounced annual inspection. The licensee was not at home at the time of arrival.
Upon arrival, LPA observed two adults supervising 11 children, which included 1 infant, 8 preschoolers, and 2 school-age children. The school-age children were the licensee's daughters. LPA checked the Guardian website to verify present adults fingerprint clearance and found that one of them did not have clearance, and the other was not associated with the facility. A Type A violation will be cited, and a civil penalty will be assessed.

The licensee, Tingling Wu, arrived at 3:07 p.m. and stated that she had left the facility at 2:00 p.m. and she provided a proof of the class schedule she is taking at the library.
During the inspection, the licensee was in compliance with the licensing capacity. The children’s roster was reviewed and found to be complete and up to date.

The licensee rents a three-bedroom, two-bathroom home. The licensee confirmed that two adults, including herself, live in the home.
LPA verified through the Guardian Roster that all live-in adults have criminal clearance.

The facility operates Monday through Friday, from 8:30 a.m. to 5:30 p.m.

Daycare Areas: Living room, kitchen, children's bathroom, and backyard.
Off-Limits Areas: Bedrooms #1, #2, and #3.

LPA also provided information about the MyChildCarePlan.org website, a consumer education resource that connects families with child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Incidental Medical Services (IMS) policy was discussed with the licensee. For additional information, LPA referred the licensee to PIN 22-02-CCP. LPA informed the licensee that when any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Information regarding ADA compliance was also shared, including the toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY). A publication titled “Commonly Asked Questions about Child Care Centers and the ADA” is available at https://www.ada.gov/resources/child-care-centers/.

LPA discussed AB 1207 with the Licensee. As of January 1, 2018, all staff must complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com.

Type A violation was issued for having S2 in the facility without a fingerprint clearance. During today's visit, the facility was issued a $100 civil penalty.
Type A violation was cited to have children in off-limit area and--no one was supervising them.
Two Type B violations were issued for chemicals and dangerous items in children’s reach and for personnel who did not have CPR training being left alone with children.
A Technical Violation was issued for expired Mandated Reporter training.
A Technical Violation was issued for not documenting the emergency drills.

LPA informed licensee, Tinglin Wu that this report dated 1/31/2025 documents Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA informed the licensee to provide a copy of this licensing report dated 1/31/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The licensee confirmed that no children currently have allergies or require medication.

LPA reviewed the files for 6 children and found all required records, including Emergency Cards and Parental Rights forms, to be complete.
LPA reviewed the licensee’s staff file and found that the licensee’s Mandated Reporter training had expired in March 2024. A Technical Violation will be issued for this.

The licensee stated that emergency drills are conducted at least once every six months. The most recent drill was conducted on December 15, 2024, but LPA observed that the drill was not documented. A Technical Violation will be issued today.

The licensee had the License and Floor Plan posted, but the Parent’s Rights Poster was not displayed. The licensee printed and posted it during the LPA visit.
The licensee confirmed that the facility carries liability insurance for the daycare and provided a copy for placement in the physical file.

The licensee was reminded that all adults 18 years of age or older living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption as specified in Health and Safety Code Section 1596.871. This must occur prior to initial presence in a licensed Family Child Care Home. LPA informed the licensee that a civil penalty of $100 per person per day will be assessed for unqualified adults present for a maximum of five days. If this is a repeat violation, the penalty may increase to $500 per person per day after the fifth day.

LPA discussed the safe sleep regulations with the licensee and shared the Child Care Licensing Safe Sleep webpage located at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the licensee about the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov. LPA recommended that the licensee register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA observed 3 children in off-limit area and no one was supervising them. The licensee stated two of them were her own children and they sometimes stay there. AType A Violation will be cited on LIC809D page.

The home was observed to be clean and in good repair, with proper temperature and ventilation.
LPA observed that the daycare area had working smoke detectors, carbon monoxide detectors, and a fully charged 2A:10BC fire extinguisher in the kitchen.

The facility has a functioning telephone, and contact information was verified.
The licensee confirmed there are no firearms or weapons in the home.
The licensee stated that breakfast, lunch, and snacks (morning and afternoon) are provided, along with drinking water.

LPA and the licensee inspected the childcare areas for potential health and safety hazards. The daycare area was clean, and all outlets were covered. However, LPA observed chemicals and a knife in a kitchen cabinet and a drawer with a broken latch. LPA discussed this with the licensee, who immediately moved these items to an inaccessible area and stated she would replace the broken latch by next week and send proof to LPA via email. A Type B violation will be issued today.

The daycare area was equipped with age-appropriate furniture, toys, books, and educational materials.
The facility currently has one infant who sleeps in a crib. LPA observed no toys, blankets, or other items were placed in or hanging over the crib. The licensee was informed about maintaining a sleep log to check on infants every 15 minutes during naptime. The licensee confirmed that no baby walkers, bouncers, or similar items are used during daycare hours.

The bathroom was clean and sanitary, with no chemicals within the children’s reach.
The outdoor play area was inspected and found to be free from health and safety hazards. There were sufficient toys and play structures for the children.

The licensee has a designated isolation area for ill children in the corner of the daycare room, where they wait for their guardians to arrive.

During the inspection, the licensee confirmed that there were no registered sex offenders living in the facility. LPA completed the RSO profile in FAS.

The appeal rights were given to the licensee.



A notice of site visit was provided and must remain posted for 30 days.
