Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi and Community Care Licensing Division Licensing Program Analyst Catrina Quimbo (LPA) met with Applicant Geoff Choy for the purpose of an announced pre-licensing inspection at 2575 Flores Street, San Mateo, CA 94403.

Applicant requests a preschool component to serve 48 children from age two years to entry into first grade. The program will operate Monday through Friday from 8:00 a.m. to 5:30 p.m. The facility will give parents the option of opting into a monthly meal plan to include breakfast, lunch and afternoon snacks. If parents choose not to opt in, they will be responsible for providing their child with breakfast, lunch and afternoon snacks as stated by applicant. The fire clearance (STD850) was granted on 6/18/2024 with both E and I4 occupancy codes, however San Mateo Consolidated Fire Inspector Jose Andrade stated facility has no restrictions for occupancy.

LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

INDOOR ACTIVITY SPACE:
There is one preschool classroom located in the facility. LPAs measured the classroom which includes a total of 2,192 square feet and will accommodate the applicants request for 48 preschool children. LPAs observed a sufficient amount of equipment, toys, tables, chairs, napping cots, and cubbies in the room. There is one first aid kit located in the main classroom. Medications will be stored in a locked cabinet in the food preparation area. LPAs observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. Applicant stated drinking water will be provided via a drinking fountain and parents will provide drinking water as well. LPAs observed a functional carbon monoxide detector in the main classroom. LPAs observed a fire extinguisher and smoke detectors in the main classroom as well. Applicant stated they will be using a paper sign-in and sign out process. Applicant stated parents will provide sheets, blankets, and pillow cases for their children during naptime. There are two restrooms for children located in the facility with 2 toilets and 1 sink in each restroom, and 4 sinks for children located in the main classroom, with a total of 4 toilets and 6 sinks available for children. Applicant stated changing area will be located next to the staff bathroom area. Applicant stated there will be a dedicated phone line for the facility. Applicant stated the staff office will serve as the isolation area for ill children. Applicant was informed facility shall be kept clean, safe, sanitary and in good conditions at all times to ensure the safety and well being of children, employees and visitors. LPAs observed a separate private restroom for the staff located next to the children’s restroom.

OUTDOOR ACTIVITY SPACE:
There is one outdoor play area on the property located behind the facility. The outdoor play area is fenced with both a concrete wall and wooden fences that are at least four feet tall. LPAs observed a play structure for ages 3 to 10 with a slide with an approximate slope grade of 40%. Applicant was advised to inspect play structure for nails, staples and any potential hazards. LPAs observed sandboxes and the applicant was reminded that sandboxes shall be inspected daily and kept free of hazardous foreign materials. Applicant stated they will add additional stability to the sandboxes via netting and sand. LPAs observed a sufficient amount of equipment and toys. There are no bodies of water on the premises. There is sufficient shaded areas provided by two large umbrellas. There is one emergency exit located in the outdoor play area, applicant stated they will provide an alarm system to the exit for additional security. LPAs advised applicant more resilient padding may be provided around play structure. LPAs advised applicant to be aware of nuts and bolts located on the wooden fence area.

LPAs measured the outdoor activity space. The outdoor play area contains a total of 1,188 square feet, which will not accommodate Applicant’s request for 48 preschool children. The outdoor area will accommodate 15 preschool children at a time.

LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days. LPAs informed Applicant lead water testing is required within 180 days of licensure.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

Based on fire clearance, the maximum number facility can be licensed for is 48 preschool children.

Prior to licensure, following must be completed:
-Outdoor rotation waiver submission and approval.
-Sound alarm installation on emergency exits located in classroom and in the outdoor area.
-Completion of securing sandboxes in outdoor area.
-Completion of securing play structure in outdoor area.
-Removal of plank boards in outdoor area.
-Submission of license for 414004729.

Exit interview conducted and report was reviewed with the Applicant, Geoff Choy.
