25 | Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi and Community Care Licensing Division Licensing Program Analyst Catrina Quimbo (LPA) met with Applicant Geoff Choy for the purpose of an announced pre-licensing inspection at 2575 Flores Street, San Mateo, CA 94403.
Applicant requests a preschool component to serve 48 children from age two years to entry into first grade. The program will operate Monday through Friday from 8:00 a.m. to 5:30 p.m. The facility will give parents the option of opting into a monthly meal plan to include breakfast, lunch and afternoon snacks. If parents choose not to opt in, they will be responsible for providing their child with breakfast, lunch and afternoon snacks as stated by applicant. The fire clearance (STD850) was granted on 6/18/2024 with both E and I4 occupancy codes, however San Mateo Consolidated Fire Inspector Jose Andrade stated facility has no restrictions for occupancy.
LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
INDOOR ACTIVITY SPACE:
There is one preschool classroom located in the facility. LPAs measured the classroom which includes a total of 2,192 square feet and will accommodate the applicants request for 48 preschool children. LPAs observed a sufficient amount of equipment, toys, tables, chairs, napping cots, and cubbies in the room. There is one first aid kit located in the main classroom. Medications will be stored in a locked cabinet in the food preparation area. LPAs observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. Applicant stated drinking water will be provided via a drinking fountain and parents will provide drinking water as well. LPAs observed a functional carbon monoxide detector in the main classroom. LPAs observed a fire extinguisher and smoke detectors in the main classroom as well. Applicant stated they will be using a paper sign-in and sign out process. Applicant stated parents will provide sheets, blankets, and pillow cases for their children during naptime. There are two restrooms for children located in the facility with 2 toilets and 1 sink in each restroom, and 4 sinks for children located in the main classroom, with a total of 4 toilets and 6 sinks available for children. Applicant stated changing area will be located next to the staff bathroom area. Applicant stated there will be a dedicated phone line for the facility. Applicant stated the staff office will serve as the isolation area for ill children. Applicant was informed facility shall be kept clean, safe, sanitary and in good conditions at all times to ensure the safety and well being of children, employees and visitors. LPAs observed a separate private restroom for the staff located next to the children’s restroom.
