On March 10, 2026, at approximately 9:30am, Licensing Program Analyst (LPA) Tapia-Mandujano conducted an unannounced plan of correction (POC) and an Increase of Capacity inspection. LPA met with licensee, Sonia Martinez Campos. Present in the home are licensee and assistant caring and supervising two infant children. Facility is operating within capacity ratios at this time. All adults living and working at the facility are fingerprint cleared and associated.

Licensee rents duplex home (B), which is a three bedroom one bathroom house with front and backyard. Licensee lives next door to another licensed facility (414004467). Licensee lives in home with adult husband and two minor children. The hours of operation are Monday-Friday from 8am-5:30pm. Daycare areas are: Living Room, Dining Room, Bathroom #1, Bedroom #2 for sleeping only, Bedroom #3 for pass through only to the backyard, and backyard. OFF limit areas are: Kitchen, Bedroom #1, Bedroom #3, Laundry Room, and front yard. All off limit areas are properly barricaded.

On February 25, 2026, LPA conducted a Case Management inspection for an increase of capacity application that was requested by licensee. During the inspection, LPA observed that the facility was operating over capacity with 6 children (4 infants and 2 preschoolers). LPA issued a Type A citation for operating over licensed capacity limits. A POC was developed with licensee and capacity limits were explained.

During today’s inspection, LPA observed that the facility is operating within capacity limits with 2 infants present. During walk through of facility a preschool age child arrived. Present at the facility were now 3 children (2 infants and 1 preschooler).

LPA conducted a health and safety inspection. LPA observed there is a fully charged fire extinguisher, and a working telephone. The fire Alarm is set up in the living room wall next to the main door. Emergency Exits are the main door and the through the left side yard. There are no pools, spas, or bodies of water on the property. Per Licensee, there are no weapons or firearms in the home. Per licensee, there is a pet cat in the home.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During today's visit, LPA observed that the report dated 02/25/26 was posted with the Notice of Site Visit. Per licensee, she also texted each parent the report and the Acknowledgement of Receipt of Licensing Report (LIC 9224). During today’s inspection, LPA observed that there were four Acknowledgement of Receipt of Licensing Report (LIC 9224) signed. Licensee was able to receive the last Acknowledgement of Receipt of Licensing Report (LIC 9224) form signed during drop off of child, making all the forms now completed.

On February 25, 2026, LPA did not approve large license as licensee was out of compliance by operating over the capacity limits.

Fire Clearance was received by Menlo Park Fire Department on January 9, 2026. Fire clearance has a restriction that access through the neighbor yard must be maintained at all times.

Capacity limits of a Small and Large Family Child Care Home were explained. Licensee has a permanent assistant. LPA explained that when no assistant is present, licensee must operate as a Small Family Child Care License.

The licensee provided proof of control of property. The licensee has not obtained a signed Property Owner/Landlord Consent form (LIC 9149). Without this consent, the licensee understands that, once licensed, they can operate with a maximum capacity of 12 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 14 children.

LPA reminded licensee that she is the sole responsible individual of this Family Child Care Home. LPA also reminded licensee that other licensed providers cannot be living, present and/or working in licensee's home during operational hours. Licensee acknowledges that children will not be exchanged, shared, or moved within other licensed family childcare homes, and enrolled children in licensee’s family child care home are to stay at this address only.

LPA will approve Large License Family Child Care Home as of today March 10, 2026.

Deficiencies were not observed today under CCR, Title 22, Div. 12, Chapt. 1.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed and translated into Spanish to licensee, Sonia Martinez Campos.
