On July 31st, 2025, Licensing Program Analyst (LPA) Maria Olguin-Leon conducted an unannounced Case management visit due to deficiencies observed during a complaint investigation. LPA met with Owner Annagi Liles and explained the purpose of the inspection.

Facility is a single license that offers a toddler and preschool program. Present in the facility today were Owner Annagi Liles and 3 staff supervising 19 preschoolers and 5 toddlers. LPA verified that all adults present during inspection have criminal background clearances and are associated to facility.

During today’s visit, LPA observed a child (C1) alone in classroom #2. LPA was in classroom #3 conducting an interview. There is an accordion door divider between classroom #2 and #3. LPA observed C1 standing alone in classroom #2 and then walked away behind accordion doors. LPA could hear C1 crying. LPA walked into classroom #2 and observed C1 crying at a corner table with C1’s head down. LPA observed there was no staff in classroom #2 and C1 was left without direct visual supervision from staff. C1 had walked into classroom #2 from outside. LPA walked over to check in on C1 and then LPA walked C1 outside to staff who were outside supervision children. LPA informed Owner that C1 was inside classroom unsupervised. Throughout the visit LPA observed staff walking from classroom to classroom leaving children without visual supervision. Based on observations during today’s visit, LPA cited a type “A”, as this poses an immediate health, safety and personal rights risk to children in care. Civil penalty was assessed.

During file review, LPA observed a 37-month-old child in the toddler program, toddler program age limitations are 18-36 months. The facility was operating beyond the conditions and limitations of their license. A type “A” deficiency was issued.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA also observed a child enrolled in the preschool program in the toddler classroom during visit, child did not have written permission from the child's authorized representative to participate in the toddler program. During nap/rest time, LPA observed children being forced to stay on mats and told to be still, close eyes and not allowed to move, as these poses a potential health, safety and personal rights risk to children in care, two type “B”’s deficiencies were cited.

Please see page LIC809D for two type “A” deficiency and two type “B” deficiency issued today under California Code of Regulations, Title 22, Division 12.

LPA Olguin-Leon informed Owner, Annagi Liles that this report dated July 31, 2025, document(s) two Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Olguin-Leon informed the Owner, Annagi Liles to provide a copy of this licensing report dated July 31, 2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements may result in an immediate civil penalty of $100. This report is public and can be reviewed.

Exit interview was conducted and report was reviewed with Owner Annagi Liles. Appeal Rights were provided.

Owner will meet with staff and discuss providing 100% supervision at all times. Owner will discuss providing visual supervision and account for children during transitions time. Based on observation, LPA observed a child (C1) walk into classroom #2 from outdoors and was crying at corner of classroom without the direct visual supervision of staff. During visit LPA observed staff walking from class to class leaving children without visual supervision. This poses an immediate health, safety and personal rights risk to children in care.

Facility was also commingling preschool age children with toddler age children which poses an immediate health, safety or personal rights risk to persons in care.

101216.4 PRESCHOOL PROGRAM WITH TODDLER COMPONENT
(a) Licensees serving preschool-age children may create a special program component for children who are between 18 months and 36 months of age. The provisions of Sections 101151 through 101239.2 shall apply for children over 24 months, in addition to those specified in Health and Safety Code section 1596.955. The provisions of Sections 101351 through 101439.1 shall apply for children between the ages of 18 and 24 months participating in a preschool toddler component, in addition to those specified in Health and Safety Code section 1596.955. (1) A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative.
