The outside play area/ patio was reviewed during inspection. The patio area was completely enclosed with tall fencing. The outside playthings inspected were in like-new condition. The facility did not have pools, ponds, or other bodies of water on the premise.
Per licensee, outside seating is used as primary activity space.
During inspection, LPA reviewed facility records including 11 children’s files and 2 personnel files. The children’s files were reviewed and included the: Identification and Emergency Information (LIC700); Consent for Medical Treatment (LIC627); Notification of Parent Rights (LIC995A); and Immunization records.
At 3:00PM., Based on record review and interview, LPA confirmed child (C1) file was missing from facility records. During inspection, licensee obtained immunization records from guardian. An advisory Note: Technical Violation (LIC9102TV) was issued to provider.
The licensee presented LPA with logs, confirming 15- minute visual review of napping infants in care. The sample log included: child’s: name, current date, and time of review.
The staff files were reviewed and included the: Notice of employee rights (LIC9052); and Staff’s Mandated Reporter Training Course (AB1207).
LPA reminded licensee to ensure the staff’s proof of required immunization are stored in the facility records. An advisory note: technical violation (LIC9102TV) was issued to provider.
The licensee's proof of current pediatric cardiopulmonary pulmonary resuscitation / first aid course was on file, and expires: 7/2027.
LPA reminded licensee to renew mandated reporter training certification, AB1207, which has expired the month prior. An Advisory Note: Technical Violation (LIC9102TV) was issued to provider.
Per licensee, facility conducts required emergency disaster drill (fire and earthquake drill) every 6 months, with last drill logged for: 7/1/2026.
The required forms are posted in entry and include Facility License Certificate and Notification of Parent’s Rights (PUB379). Per license, isolation of an ill child is in the playroom. Per licensee, the childcare service provides daily snack and lunch for children. (REFER TO 809C, FOR CONT.)
