13 | On June 29, 2026, at 8.30AM, Licensing Program Analyst (LPA) Tso conducted an unannounced visit for delivery of the complaint investigation findings and met with the Teacher, Vivien Qin. LPA explained the purpose of the inspection and were granted entry to the facility by the Teacher. Present, a teacher was supervising 2 children.
Based on LPA’s gathered information through observation, interviews and records reviews, the agency has investigated the complaint allegation above. The facility did not comply with the criminal records clearance regulations. A staff obtained the criminal records clearance but did not associate to the facility. The preponderance of evidence standard has been met. The above allegation is found to be SUBSTANTIATED. A Type B violation was issued today, June 29, 2026 in accordance with the California Code of Regulations, Title 22, Division 12, citation was being cited on the attached LIC 9099D. Since the criminal records clearance of the staff was associated to the facility on 4/29/2026. The deficiency was cleared on today, June 29, 2026 visit.
Exit interview conducted and a copy of this report and appeal rights were discussed and left with the Teacher, Vivien Qin, whose signature on this form confirms receipt of these reports. Notice of Site Visit (The Notice) was provided. The Notice is to remain posted for 30 days.
