On November 5, 2025 at approximately 9:00am, Licensing Program Analysts (LPAs) Catrina Quimbo and Leslit Tapia-Mandujano, conducted a follow up pre-licensing visit to Redwood City Child Development Program. LPAs met with facility director, Mary Lisa Penisini, and executive director, Kiri Fluestch, and explained the purpose of the visit. Present during LPAs' visit included 47 children with 14 teaching staff. Teaching staff present have fingerprint clearance on file.

Program is currently licensed for a preschool component (under license 410509730) and an infant component (under license 410509731).

Executive director requested to add a toddler component. This is a single license conversion application. Applicant requested for a total capacity of 86 children: 8 infants (ages 0 to 24 months), 12 toddlers (ages 18 months to 36 months) and 66 preschoolers (ages 2 years to 5 years old).

The facility has 1 infant classroom (Joeys), 1 toddler classroom (Koalas), and 3 preschool classrooms (Polar Bears, Panda Bears, and Brown Bears/Modular class).

An initial fire clearance was received April 30, 2025 for 20 children total (12 toddlers and 8 infants). A second fire clearance was received August 13, 2025 for 86 children total (66 preschoolers, 12 toddlers and 8 infants).

An initial pre-licensing was conducted August 12, 2025. LPAs had measured all infant, toddler and preschool classrooms to verify square footage was sufficient per requested capacity. During initial pre-licensing inspection, the infant component's napping space was not per Title 22 regulations. Applicant was to reconfigure infants' napping space to be within compliance of regulations.

A third fire clearance was received October 16, 2025 for 86 children (66 preschoolers, 12 toddlers and 8 infants). Per fire clearance conditions, the infant room's maximum occupancy is for 11 individuals (3 staff and 8 children). Per fire clearance conditions, the Brown Bears maximum occupancy is for 24.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During today's visit, LPAs toured and inspected all infant, toddler and preschool classrooms. Applicant reconfigured napping space in the infant classroom. LPAs remeasured the infant and Brown Bear/Modular classrooms during today's visit.

Infant classroom includes a separate napping space, designated for infants who sleep in cribs. Napping space is physically separate from infants' indoor activity space. Partitions separating the napping and activity space are at least 4 ft. high and is of sound absorbing materials. LPAs observed partitions to be secured to the walls. LPAs also observed crib to be positioned in napping space that does not hinder the entrance or exit to the classroom.

There are cots available for infants who are capable of climbing out of a crib. LPAs observed cots to be labeled with children's individual names.

Director and executive director plan to utilize the designated area for cribs when infants who are required to sleep in cribs, are enrolled. When there are no infants who sleep in cribs enrolled in program, director and executive director plan to utilize the designated crib area as an additional "cozy corner"/ quiet area for infants.
LPAs discussed with director and executive director that when an infant is sleeping in a crib, the designated crib area is to be used for cribs exclusively. LPAs discussed that when there are no infants who utilize a crib for napping, the designated crib area can be used as a "cozy corner"/ quiet area exclusively. LPAs discussed that the designated napping area for cribs cannot be used as an activity space for infants simultaneously.

Infant indoor measurements are:
The infant indoor measurements contain a total of 299.59 square feet which will accommodate applicant's request for 8 infants.

Toddler Indoor measurements are:
The toddler indoor measurements contain a total 427.29 square feet, which will accommodate applicant's request for 12 toddlers.

Preschool Indoor measurements are:
The preschool indoor measurements contain a total of 2,205.72 square feet which will not accommodate applicant's request for 66 preschoolers.

LPAs did not observe changes to outdoor areas from LPAs' initial pre-licensing visit. Outdoor areas were not remeasured during today's visit.

LPAs discussed with facility director and executive director all physical plant areas are to remain as is. Prior to any changes being made to the facility, the department must be made aware. Department must also approve of changes prior to usage. Per Title 22, Section 101212(c) The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space.

Based on fire clearance conditions, Brown Bears/Modular maximum occupancy is for 24. Based on this condition, maximum capacity program will be licensed for 81 children (8 infants, 12 toddlers, and 61 preschoolers).

Prior to licensure, following is to be completed:
-Updated LIC200A with updated capacity of 81 children: 8 infants, 12 toddlers, and 61 preschoolers.
-Updated fire clearance with updated capacity of 81 children.
-Ensuring the wall partitions in infants' napping area, are more firmly secured.
-Centralized Application Bureau (CAB) final application approval.

Facility numbers 410509730 and 410509731 will be permanently closed per CAB's approval.

Applicant, Kiri Fluetsch, understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
